Enteric fever, often referred to as typhoid or paratyphoid fever, is a life-threatening illness caused by Salmonella Typhi and Salmonella Paratyphi. With global incidence rates remaining significant, it is crucial for healthcare professionals to be equipped with the latest knowledge on its diagnosis and management.
Diagnosis of enteric fever involves a combination of clinical suspicion, based on patient history and symptoms, and laboratory confirmation. Blood cultures remain the gold standard for diagnosis, with a sensitivity of 40-80%. However, the advent of molecular methods, such as polymerase chain reaction (PCR), has improved diagnostic accuracy. PCR can detect S. Typhi and S. Paratyphi DNA in blood, stool, and bone marrow samples, offering a rapid and sensitive method.
Increasing antibiotic resistance has complicated the management of enteric fever. Multi-drug resistant (MDR) strains, resistant to ampicillin, chloramphenicol, and trimethoprim-sulfamethoxazole, are widespread. Fluoroquinolones, once first-line therapy, are now less effective due to widespread resistance. Current guidelines recommend empiric therapy with third-generation cephalosporins or azithromycin, pending culture results. Treatment should be guided by local susceptibility patterns and individual patient factors.
Vaccination remains a key strategy in the prevention of enteric fever. Two vaccines, Vi polysaccharide vaccine (ViPS) and Ty21a, are currently available. However, their use is limited by moderate efficacy and short duration of protection. Newer conjugate vaccines, such as Typbar-TCV®, show promise with higher efficacy and longer duration of protection. Prevention also involves ensuring safe water and food practices, particularly in endemic areas.
In conclusion, the diagnosis and management of enteric fever continue to evolve in response to changing epidemiology and antibiotic resistance patterns. Continued vigilance, appropriate use of diagnostic tests, adherence to updated treatment guidelines, and effective vaccination strategies are vital in controlling this significant global health threat.
