Abnormal liver function tests are a common issue faced by clinicians. Elevated liver enzymes often pose a diagnostic challenge, requiring a systematic approach to identify the underlying cause and develop an effective management plan.
Liver enzymes are proteins that facilitate numerous biochemical reactions in the liver. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) are the most commonly evaluated enzymes. Values above the upper limit of normal (ULN) suggest hepatic injury or disease.
A comprehensive history and physical examination are indispensable in the diagnostic process. It is crucial to identify risk factors such as alcohol consumption, medication use, viral hepatitis exposure, and family history of liver disease. Laboratory investigations should include a complete blood count, coagulation profile, serologic tests for viral hepatitis, and imaging studies if necessary.
Management of elevated liver enzymes depends on the underlying cause. Lifestyle modifications, including alcohol cessation and weight loss, can be beneficial in cases of alcoholic liver disease and nonalcoholic fatty liver disease. Antiviral therapy is indicated for chronic viral hepatitis. For drug-induced liver injury, discontinuation of the offending drug is usually sufficient.
Regular monitoring of liver enzymes is essential to assess the response to treatment and detect potential complications. If the enzymes remain persistently elevated despite appropriate management, further investigations, including liver biopsy, may be required.
Addressing elevated liver enzymes necessitates a comprehensive, patient-centered approach. By understanding the potential causes and implementing an effective diagnostic and management plan, clinicians can ensure optimal outcomes for their patients with liver disease.
