A thorough history and physical examination are fundamental to the assessment of abdominal pain. The physician should note the onset, location, duration, character, and radiation of pain, along with associated symptoms. The physical examination should focus on identifying signs of acute abdominal conditions that may require emergent intervention.
Based on the initial assessment, appropriate diagnostic tests should be ordered. Laboratory tests, including a complete blood count, liver function tests, and lipase level, can provide valuable information. Imaging studies, such as ultrasound, computed tomography, or magnetic resonance imaging, may be necessary to further delineate the cause of pain.
Special populations, such as the elderly, pregnant women, and immunocompromised patients, require careful consideration due to altered physiological states and unique disease presentations. For instance, abdominal pain in a pregnant woman may be due to obstetric complications and requires specialized evaluation.
Management should be tailored to the underlying etiology. Non-specific, functional, or chronic abdominal pain may benefit from lifestyle modifications and pharmacological therapy. Acute surgical conditions necessitate prompt referral to a surgeon.
Evaluating abdominal pain in clinical practice is a complex task that requires a comprehensive, systematic approach. A thorough history and physical examination, appropriate diagnostic testing, and consideration of special populations form the cornerstone of this evaluation. Tailored management strategies based on the underlying etiology can help alleviate pain and improve patient outcomes.
