This piece aims to discuss strategies for the timely identification and initial management of sepsis in clinical practice.
Recognizing sepsis early is crucial to improving patient prognosis. Healthcare professionals must maintain a high index of suspicion, particularly in patients with known infections, immunocompromised states, or recent surgical procedures. Key indicators such as altered mental status, tachycardia, tachypnea, and fever should trigger further evaluation. The use of validated screening tools, such as the quick Sequential Organ Failure Assessment (qSOFA) score, can aid in early recognition.
Once sepsis is suspected, immediate intervention is essential. The "sepsis bundle," recommended by the Surviving Sepsis Campaign, provides a structured approach. This includes the administration of broad-spectrum antibiotics within one hour of diagnosis, fluid resuscitation, and vasopressors for refractory hypotension. Source control, such as surgical drainage of abscesses or removal of infected devices, should be performed promptly.
Management of sepsis requires a multidisciplinary approach. Involvement of critical care, infectious disease, pharmacy, and nursing teams can optimize patient care. Regular team huddles, checklists, and sepsis pathways can facilitate communication, streamline care, and ensure adherence to best practices.
