Antimicrobial stewardship programs (ASPs) are integral in optimizing the use of antimicrobials, thereby improving patient outcomes, reducing microbial resistance, and decreasing the spread of infections caused by multi-drug-resistant organisms. The successful implementation of these programs in hospitals is a crucial step in combating the global threat of antimicrobial resistance.
Establishing an interdisciplinary team is the cornerstone of an effective ASP. This team, led by an infectious disease physician and a clinical pharmacist with infectious diseases training, should also include microbiologists, infection preventionists, and information system specialists. Regular meetings should be held to review antimicrobial use and resistance data, and to develop strategies for improvement.
Prospective audit and feedback is a powerful strategy for modifying prescriber behavior. Regular reviews of antimicrobial prescriptions with feedback to the prescriber can improve patient outcomes and reduce unnecessary antimicrobial use. Another key strategy is formulary restriction and preauthorization, which can lead to immediate reductions in antimicrobial use and cost. However, this strategy requires significant resources and may be met with resistance from prescribers.
Education and training are critical to the success of an ASP. Regular educational sessions can improve prescribers’ knowledge of antimicrobial resistance and stewardship principles. Additionally, training can help to develop the necessary skills to implement and maintain the ASP.
The effectiveness of an ASP should be regularly evaluated using specific process and outcome measures. These measures can include antimicrobial use metrics, resistance patterns, and patient outcomes. Regular feedback of these results to healthcare providers can drive continuous improvement.
Implementing an effective ASP in hospitals is a complex but necessary task. It requires a multidisciplinary team, effective strategies, education, and ongoing evaluation. With commitment and resources, hospitals can successfully implement ASPs and contribute to the global fight against antimicrobial resistance.
