Unexplained weight loss (UWL), defined as a decrease of 5% or more in body weight over a period of 6-12 months without a known cause, is a challenging clinical scenario. It can be indicative of a myriad of underlying conditions, ranging from malignancies to chronic infections, and therefore demands a comprehensive, systematic approach.
The first step in the assessment of UWL is a thorough history and physical examination. Special attention should be given to dietary intake, physical activity, psychological status, and medication use. The presence of symptoms like fever, pain, or changes in bowel habits might suggest specific etiologies.
Basic laboratory investigations, including complete blood count, renal and liver function tests, thyroid function tests, and markers of inflammation, are usually conducted in all patients. Imaging studies, such as chest X-ray or abdominal ultrasound, may be considered based on the patient's symptoms and risk factors. More targeted investigations, like endoscopy or biopsy, may be needed if initial tests are inconclusive.
Management strategies for UWL depend on the underlying cause. In some cases, symptom management and nutritional support may be all that's required. In others, specific treatments for identified diseases are necessary. It's crucial to involve a multidisciplinary team, including dietitians and psychologists, in the patient's care.
The approach to UWL in clinical practice requires careful evaluation, a high index of suspicion, and a patient-centered approach. By combining a thorough history, physical examination, and appropriate investigations, clinicians can effectively diagnose and manage the underlying causes of UWL. Continued research is needed to improve our understanding and management of this complex clinical scenario.
