Unexplained weight loss (UWL) is a common yet challenging scenario in clinical practice. It often signifies underlying disease, and its diagnostic approach requires a comprehensive understanding of potential etiologies. This article will elucidate a systematic approach to diagnosing UWL.
UWL can be a manifestation of various illnesses, including malignancies, endocrine disorders, gastrointestinal diseases, psychiatric conditions, and infectious diseases. A detailed history and physical examination can often guide clinicians towards the likely cause. However, in approximately 15-25% of UWL cases, the cause remains unexplained after initial evaluation.
The diagnostic approach to UWL begins with a thorough history and physical examination, focusing on symptoms suggestive of specific diseases. Laboratory investigations should include complete blood count, liver and renal function tests, thyroid function tests, and HIV testing. If initial investigations are unremarkable, further tests such as CT scan, upper and lower GI endoscopy, and psychiatric evaluation may be considered based on individual clinical scenarios.
Imaging plays a crucial role in diagnosing UWL, particularly in identifying malignancies. CT scan is the most common imaging modality used, but PET scan may be considered in cases where malignancy is suspected but not confirmed on CT scan.
Follow-up is critical in managing patients with UWL. Even if the initial evaluation is unremarkable, patients should be closely monitored for the development of new symptoms or signs that may point to a specific diagnosis. In some cases, the cause of UWL may become apparent only after a period of observation.
UWL is a complex clinical problem that requires a comprehensive and systematic approach. While a detailed history and physical examination are the cornerstones of evaluation, laboratory investigations, imaging, and close follow-up are also crucial in diagnosing the underlying cause. By adopting this comprehensive approach, clinicians can ensure early detection and treatment of potentially serious conditions underlying UWL.
