Abdominal pain, a common presenting symptom in clinical practice, often poses a diagnostic challenge due to its multifaceted etiology. This article aims to provide a comprehensive approach to evaluating abdominal pain, enabling a more accurate and efficient diagnosis.
The initial assessment should focus on identifying life-threatening causes. The clinician should consider the patient's age, medical history, and the nature of the pain. Vital signs and physical examination can provide clues to the severity and potential causes. A detailed history, including the onset, location, and characteristics of the pain, should be obtained.
Based on the initial assessment, appropriate laboratory and imaging studies should be ordered. Routine tests may include complete blood count, liver function tests, and urinalysis. Imaging studies such as ultrasound, computed tomography (CT), or magnetic resonance imaging (MRI) can provide further insights, especially when the cause is not apparent from the history and physical examination.
Special populations may require a different approach. In women, gynecological causes should be considered. In elderly patients, vascular causes are more common. In patients with a history of surgery, adhesions should be considered. In all cases, a high index of suspicion for serious causes should be maintained.
If the diagnosis remains elusive after initial evaluation, referral to a specialist may be necessary. Regardless of the cause, appropriate follow-up is crucial to ensure symptom resolution and to monitor for any potential complications.
Abdominal pain evaluation requires a systematic approach, considering the broad differential diagnosis. By maintaining a high index of suspicion for serious causes and tailoring the evaluation to the individual patient, clinicians can improve diagnostic accuracy and patient outcomes.
