The initial evaluation of acute chest pain involves a thorough history taking and physical examination. The clinician should focus on the character, onset, location, duration, and radiation of the pain. Associated symptoms such as shortness of breath, nausea, and diaphoresis can suggest a cardiac etiology.
Diagnostic testing is paramount in the evaluation of acute chest pain. An electrocardiogram (ECG) should be obtained immediately to identify ST-elevation myocardial infarction (STEMI). Serial cardiac biomarkers, chest x-ray, and echocardiogram can further assist in the diagnosis.
Risk stratification using tools such as the HEART score aids in determining the appropriate disposition of the patient. This score incorporates History, ECG, Age, Risk factors, and Troponin levels to predict major adverse cardiac events.
Management strategies depend on the underlying cause of the chest pain. In cases of suspected acute coronary syndrome (ACS), initial management includes aspirin, nitroglycerin, and possibly anticoagulation. Refractory cases may require invasive strategies such as percutaneous coronary intervention (PCI).
Acute chest pain is a common but challenging presentation in clinical practice. A structured approach involving thorough history taking, appropriate diagnostic testing, risk stratification, and tailored management can ensure optimal patient outcomes. Continual education and staying abreast with current guidelines are crucial for healthcare professionals dealing with such cases.
