Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, is a significant global health concern, especially in developing countries. The increasing antibiotic resistance and changing epidemiology of this disease necessitate a comprehensive understanding and improved management strategies amongst healthcare professionals.
Enteric fever is endemic in regions with poor sanitation and inadequate access to clean water. The World Health Organization estimates 11-20 million cases annually, with about 128,000 to 161,000 deaths. Travel to endemic regions, consumption of contaminated food or water, and close contact with an infected person are prominent risk factors.
Clinical presentation varies from mild illness with low-grade fever to severe disease with complications such as intestinal perforation and neurological manifestations. Blood cultures are the gold standard for diagnosis, although their sensitivity is limited. Rapid diagnostic tests based on antigen detection are increasingly being used, though they have variable sensitivity and specificity.
Antibiotic therapy remains the cornerstone of enteric fever management. Choice of antibiotics depends on local susceptibility patterns, with third-generation cephalosporins and fluoroquinolones commonly used. Increasing resistance to these drugs is a significant concern, necessitating regular surveillance and judicious use of antibiotics. Vaccination, improving sanitation, and access to clean water are critical for disease control in endemic regions.
Research is ongoing to develop more effective vaccines and rapid, reliable diagnostic tests. Understanding the disease's genetic basis may also help in identifying novel therapeutic targets. Antimicrobial stewardship programs and improved public health measures are crucial to control the disease spread and manage antibiotic resistance.
Enteric fever is a significant global health concern requiring comprehensive understanding and improved management strategies. Healthcare professionals need to stay updated on the latest developments in diagnosis, treatment, and prevention to provide optimal care to patients and control this disease effectively.
