Peptic ulcer disease (PUD) is a common digestive disorder, often resulting from the imbalance between gastroprotective and gastrodustructive factors. In recent years, advancements in diagnostic and therapeutic strategies have significantly improved PUD management.
Earlier, PUD diagnosis relied heavily on invasive endoscopy. However, the advent of non-invasive tests like urea breath test (UBT), stool antigen test (SAT), and serology for Helicobacter pylori detection have revolutionized PUD diagnosis. These tests, while maintaining accuracy, reduce patient discomfort and risk associated with invasive procedures.
Traditional PUD treatment centered on antacids and H2 receptor blockers. The discovery of H. pylori's role in PUD pathogenesis led to the 'triple therapy' regimen, which includes a proton pump inhibitor (PPI) and two antibiotics. However, increasing antibiotic resistance necessitated alternative strategies.
Recent advancements include 'quadruple therapy' and 'sequential therapy', which have shown improved eradication rates. Additionally, the development of potassium-competitive acid blockers (P-CABs) promises a new era in acid suppression therapy. P-CABs, with their rapid onset and potent acid suppression, could potentially replace PPIs in the future.
Recent research suggests that probiotics can play a significant role in PUD management. They can help restore gut microbiota balance, enhance the efficacy of H. pylori eradication therapy, and reduce side effects associated with antibiotic therapy. However, more extensive clinical trials are needed to establish their definitive role.
Advancements in PUD management have significantly improved patient outcomes. However, challenges like antibiotic resistance and side effects of long-term PPI use necessitate further research. The future of PUD management lies in personalized medicine, which takes into account the patient's genetic makeup, lifestyle, and microbiota, ensuring a more targeted and effective treatment approach.
