Hypertensive emergencies are acute, life-threatening conditions where uncontrolled hypertension leads to progressive or impending end-organ damage. These emergencies can manifest as hypertensive encephalopathy, acute coronary syndrome, acute renal failure, or aortic dissection, among others. Accurate diagnosis necessitates a thorough clinical examination, supported by laboratory and imaging studies.
Immediate reduction in blood pressure is crucial in hypertensive emergencies. However, overly aggressive management can lead to underperfusion and organ damage. The choice of antihypertensive therapy depends on the specific clinical scenario, with intravenous agents preferred for rapid action. Commonly used drugs include sodium nitroprusside, labetalol, and nicardipine.
Recent research has led to new insights into the pathophysiology of hypertensive emergencies, leading to improved management strategies. Novel therapeutic agents, such as clevidipine, are showing promise in controlling blood pressure rapidly and precisely.
