Fever is a common symptom encountered in clinical practice. Despite its prevalence, it is often misunderstood and mismanaged. This article aims to provide a comprehensive understanding of fever and its management.
Fever is not a disease, but a physiological response to infection or inflammation. It is a protective mechanism that enhances the body's immune response. Fever is generally defined as a temporary elevation in the body's thermoregulatory set-point, with a body temperature of 100.4°F (38°C) or higher.
Assessment of fever involves a thorough history and physical examination. The history should include the duration, pattern, and associated symptoms. Physical examination should focus on identifying the source of infection or inflammation. Laboratory investigations may be required based on the clinical suspicion.
The management of fever depends on its cause. If an infection is identified, appropriate antimicrobial therapy should be initiated. In cases of non-infectious fever, treatment of the underlying condition is required. Antipyretics can be used to alleviate discomfort, but they do not treat the underlying cause of the fever.
Antipyretics, such as acetaminophen and nonsteroidal anti-inflammatory drugs (NSAIDs), are commonly used to relieve discomfort associated with fever. However, they should be used judiciously as they can mask symptoms and delay diagnosis. Furthermore, they can have side effects, especially if used in high doses or for prolonged periods.
Understanding the nature of fever and its management is critical in clinical practice. Fever is a symptom, not a disease, and its management should focus on identifying and treating the underlying cause. Antipyretics can be used for symptomatic relief, but they do not treat the cause of the fever. Proper assessment and management of fever can improve patient outcomes and reduce the risk of complications.
