Introduction

Chronic cough, defined as a cough persisting for more than eight weeks, presents a common yet complex clinical challenge. Despite its prevalence, the diagnosis and management of chronic cough can be complex, often leading to frustration for both the patient and the clinician.

Etiology and Pathophysiology

Chronic cough is usually secondary to a variety of conditions such as gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and asthma. However, it may also be idiopathic. Understanding the underlying pathophysiology is crucial for establishing an effective treatment plan. The cough reflex involves both the central and peripheral nervous systems, with dysfunction in these systems potentially leading to chronic cough.

Diagnostic Approach

A comprehensive history and physical examination form the cornerstone of diagnosis. Clinicians should assess for common causes, consider less common etiologies, and recognise when the cough may be a symptom of a serious underlying condition. Diagnostic testing, such as chest radiography, spirometry, and bronchoscopy, may be necessary based on individual patient circumstances.

Treatment Strategies

Treatment should be tailored to the underlying cause, if identifiable. Empiric treatment trials for the most common causes of chronic cough can be considered when the diagnosis is uncertain. If the cough is unexplained or refractory to treatment, referral to a specialist may be warranted. Non-pharmacological interventions, including behavioral therapy and speech pathology, can also play a critical role in management.

Conclusion

Chronic cough is a multifaceted condition requiring a comprehensive, patient-centered approach for effective diagnosis and management. Clinicians should maintain a high index of suspicion for common and uncommon causes, and use a stepwise approach to guide diagnostic testing and treatment. Further research is needed to better understand the pathophysiology of chronic cough and to develop more effective treatment strategies.
