Thyroid disorders constitute a significant portion of endocrine pathologies, affecting millions worldwide. This article aims to provide healthcare professionals with a comprehensive understanding of adult thyroid disorders and their management.
Thyroid disorders predominantly affect women and the elderly. The primary etiologies include autoimmune disease, iodine deficiency, and genetic predisposition. Thyroid dysfunction can cause hypothyroidism, hyperthyroidism, goiter, and thyroid cancer.
Patients with hypothyroidism often present with fatigue, weight gain, and cold intolerance. Conversely, hyperthyroidism symptoms include weight loss, heat intolerance, and palpitations. Goiter and thyroid cancer may present as a neck mass, with potential compressive symptoms.
Diagnosis relies on a combination of clinical history, physical examination, and laboratory tests. Serum thyroid-stimulating hormone (TSH) level serves as the initial screening test. Abnormal TSH levels warrant further testing with free T4 and T3 levels.
Hypothyroidism is typically managed with levothyroxine replacement therapy. Hyperthyroidism treatment options include antithyroid drugs, radioactive iodine ablation, or surgery. Goiter and thyroid cancer management often involve surgical intervention.
Regular follow-up is crucial to monitor treatment efficacy and adjust dosages. Most patients with hypothyroidism and hyperthyroidism can achieve euthyroid status with appropriate treatment. However, thyroid cancer prognosis varies based on the type and stage of the disease.
Thyroid disorders are common yet complex conditions requiring a thorough understanding of their pathophysiology, clinical presentation, diagnostic approach, and management strategies. With appropriate treatment and follow-up, most patients can maintain a good quality of life.
