Unexplained weight loss (UWL) is a clinical conundrum that often presents a diagnostic challenge to physicians due to its myriad potential causes. This article aims to provide a comprehensive approach to evaluating patients presenting with UWL, facilitating accurate diagnosis and prompt treatment.
The first step in the evaluation process is a thorough history and physical examination. This includes understanding the patient's baseline weight, the duration and extent of weight loss, associated symptoms, and past medical history. A detailed dietary history, psycho-social evaluation, and medication review are also imperative.
Following the initial assessment, laboratory investigations should include a complete blood count, metabolic panel, liver function tests, thyroid function tests, and inflammatory markers. Imaging studies, such as chest X-rays, abdominal ultrasound or CT scans, may be indicated based on the patient's symptoms and clinical suspicion.
If the initial investigations are inconclusive, more specialized tests may be warranted. These could include endoscopic evaluations, capsule endoscopy, colonoscopy, or tumor markers. In some cases, a biopsy may be necessary to rule out malignancies.
While malignancies, gastrointestinal disorders, and endocrine abnormalities are common causes of UWL, atypical causes should not be overlooked. These include chronic infections, autoimmune diseases, psychiatric disorders, and social determinants of health such as food insecurity.
Evaluating unexplained weight loss is a complex process that requires a systematic and comprehensive approach. By conducting a thorough initial assessment, appropriate laboratory and imaging studies, specialized investigations if needed, and considering atypical causes, physicians can effectively diagnose and manage patients presenting with this challenging clinical picture.
