Interstitial lung diseases (ILDs) represent a heterogeneous group of more than 200 lung disorders characterized by inflammation and fibrosis of the lung parenchyma. These conditions, often progressive and potentially fatal, pose significant diagnostic and therapeutic challenges to healthcare professionals.
The pathophysiology of ILDs is complex and multifactorial, involving genetic predisposition, environmental exposures, and aberrant wound healing responses. These factors lead to alveolar epithelial injury, recruitment of inflammatory cells, and deposition of extracellular matrix, resulting in lung fibrosis and architectural distortion.
Patients with ILDs typically present with progressive dyspnea and non-productive cough. High-resolution computed tomography (HRCT) is the gold standard for diagnosis, revealing characteristic patterns such as reticular abnormalities, honeycombing, and ground-glass opacities. Pulmonary function tests often show a restrictive pattern with reduced diffusion capacity.
Management of ILDs is complex and often necessitates a multidisciplinary approach. Corticosteroids and immunosuppressants form the cornerstone of treatment for many ILDs. However, for idiopathic pulmonary fibrosis, the most common ILD, antifibrotic agents such as pirfenidone and nintedanib have shown benefit. Lung transplantation may be considered in severe cases.
The prognosis of ILDs varies widely, with median survival times ranging from 3 to 10 years depending on the specific ILD subtype. Early recognition and appropriate management can significantly improve patient outcomes. As healthcare professionals, understanding the complexities of ILDs is crucial to provide optimal care for these patients.

A thorough understanding of their pathophysiology, clinical presentation, diagnostic strategies, and management options is essential for healthcare professionals to effectively care for patients with these challenging conditions.
