Acute chest pain (ACP) is a common yet challenging presentation in emergency departments. The differential diagnosis is broad, ranging from benign conditions to life-threatening emergencies. This article aims to provide a comprehensive guide to the clinical approach to ACP, assisting healthcare professionals in making accurate diagnoses and managing patients effectively.
ACP necessitates immediate triage and evaluation. Key aspects include a detailed history, physical examination, and initial investigations. The history should focus on the character, onset, location, duration, and radiation of the pain. Associated symptoms such as dyspnea, diaphoresis, nausea, or syncope should be noted. Physical examination should assess vital signs, cardiac and respiratory systems, and potential sources of referred pain.
Electrocardiogram (ECG) is the first-line investigation for ACP. It can reveal myocardial ischemia, infarction, or arrhythmias. Serial cardiac biomarkers are also crucial in diagnosing myocardial damage. Additional investigations like chest X-ray, computed tomography (CT), or echocardiography may be required based on the clinical suspicion.
Management should be tailored to the underlying cause of ACP. For suspected acute coronary syndrome (ACS), immediate reperfusion therapy is recommended. This may include pharmacological therapy with antiplatelets and anticoagulants, or invasive strategies like percutaneous coronary intervention (PCI). Non-cardiac causes should be managed according to their specific treatment guidelines.
Risk stratification is essential to guide management and disposition of patients with ACP. Tools like the HEART score can help in assessing the risk of major adverse cardiac events. High-risk patients require intensive care, while low-risk patients may be suitable for outpatient management or further testing.
The clinical approach to ACP requires a systematic evaluation, judicious use of diagnostic modalities, and tailored treatment strategies. Risk stratification plays a pivotal role in managing these patients. Ongoing education and adherence to evidence-based guidelines can ensure optimal patient outcomes in this critical clinical scenario.
