Acute chest pain (ACP) is a common yet challenging symptom to evaluate in clinical practice due to its numerous potential etiologies. It is crucial for clinicians to accurately diagnose and manage ACP to prevent severe complications.
ACP can stem from cardiovascular, gastrointestinal, pulmonary, musculoskeletal, or psychological causes. Cardiovascular etiologies, including acute coronary syndrome (ACS), are the most critical, requiring immediate attention. Risk stratification using clinical history, physical examination, and diagnostic tests is fundamental to discern the cause.
Initial evaluation should include a thorough history and physical examination, focusing on risk factors and symptom characteristics. Electrocardiogram (ECG) and cardiac biomarkers are essential for detecting ACS. Further, imaging studies like chest radiographs or CT scans can help identify non-cardiac causes. Advanced modalities such as coronary angiography may be needed based on the patient's risk profile.
Management of ACP is guided by the underlying cause. ACS necessitates immediate revascularization strategies along with medical therapy. Non-cardiac chest pain may be managed with appropriate treatment for the underlying condition, such as antacids for gastroesophageal reflux disease or analgesics for musculoskeletal pain.
Clinical guidelines provide a structured approach to ACP. They emphasize the importance of timely diagnosis and management, and the need for patient education on symptom recognition and when to seek medical attention. Adherence to these guidelines can significantly improve patient outcomes.
Diagnosing and managing ACP requires a comprehensive and systematic approach. Understanding the various etiologies, utilizing appropriate diagnostic tools, and implementing effective treatment strategies are key to optimizing patient care. Clinicians must stay updated with the latest guidelines to ensure best practices in managing this complex clinical scenario.
