Abstract

Sustained recovery from substance use disorders (SUDs) remains a central challenge in addiction medicine. This review synthesizes contemporary evidence on longitudinal outcomes, elucidates pathophysiological underpinnings, and evaluates the efficacy of diverse treatment modalities, including pharmacotherapy, psychosocial interventions, and emerging therapies. Introduction

Addiction is a chronic relapsing disorder characterized by compulsive substance seeking, continued use despite adverse consequences, and significant neurobiological changes. Recent advances in neuropharmacology, behavioral interventions, and health systems approaches have transformed the landscape of addiction treatment. However, achieving and maintaining sustained recovery remains complex, requiring a nuanced understanding of the interplay between biological, psychological, and social determinants. Comprehensive evaluation also entails assessment of medical comorbidities, mental health status, and social determinants of health. Treatment & Management

Optimal management of SUDs requires a multimodal approach, integrating pharmacotherapy, behavioral interventions, and recovery support services. Evidence-based pharmacotherapies include opioid agonists (methadone, buprenorphine), opioid antagonists (naltrexone), and medications for alcohol use disorder (acamprosate, disulfiram, naltrexone). Current clinical guidelines from organizations such as the American Society of Addiction Medicine (ASAM), National Institute on Drug Abuse (NIDA), and World Health Organization (WHO) emphasize a chronic care model, routine use of medications for opioid and alcohol use disorders, and the importance of comprehensive psychosocial support. Guidelines stress individualized treatment planning, regular monitoring for relapse, and coordinated care across medical, psychiatric, and social domains. Harm reduction strategies, including naloxone distribution and syringe service programs, are endorsed to minimize adverse outcomes and facilitate engagement in long-term treatment.

Sustained recovery following addiction treatment is achievable through the integration of evidence-based pharmacotherapy, psychosocial interventions, and recovery-oriented support. A nuanced understanding of neurobiology, risk factors, and evolving therapeutic modalities informs clinical practice and enhances patient outcomes. Ongoing research into novel interventions and chronic disease management strategies holds promise for further improving rates of sustained remission. Multidisciplinary collaboration and guideline-driven care are essential for optimizing recovery pathways and addressing the enduring public health challenge of addiction.
