Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, continues to pose significant global health challenges. Adequate diagnosis and management are crucial to mitigate its impact on public health.
Early diagnosis of enteric fever is pivotal in reducing morbidity and mortality. Blood culture remains the gold standard for diagnosis, with a sensitivity of 40-60% in the first week of illness. PCR-based methods are promising, but their use is limited due to high costs and requirement of specialized laboratories. Serology tests like Widal test, although widely used, have low specificity and sensitivity.
Antimicrobial therapy forms the cornerstone of enteric fever management. Fluoroquinolones were once the treatment of choice, but increasing resistance has necessitated the use of alternatives. Azithromycin and third-generation cephalosporins like ceftriaxone are now first-line treatments. Antimicrobial susceptibility testing is recommended to guide therapy.
Vaccination is a key preventive measure against enteric fever. Two vaccines are currently available: the oral live-attenuated Ty21a vaccine and the injectable Vi polysaccharide vaccine. Both vaccines are moderately effective and should be considered in endemic areas and for travelers to such regions.
Beyond clinical management, public health measures including improved sanitation, safe water supplies, and food safety are crucial in controlling enteric fever. These measures, coupled with surveillance and reporting systems, can substantially reduce the disease burden.
Enteric fever remains a significant health challenge. Effective strategies encompass prompt diagnosis, appropriate antimicrobial therapy, vaccination, and public health measures. Continued research and surveillance are essential to inform practice and policy, ultimately aiming to eradicate this preventable disease.
