Acute chest pain (ACP) remains a common, yet challenging, clinical presentation. Its etiology ranges from benign conditions to life-threatening emergencies, making its evaluation critical. This article presents a comprehensive approach to diagnosing and managing ACP in clinical practice.
ACP evaluation begins with a thorough history and physical examination to identify high-risk features. Vital signs, patient's age, comorbidities, and the nature of the pain are crucial in this assessment. The HEART score, a validated tool, can guide risk stratification and further management.
Electrocardiogram (ECG) is the initial diagnostic test, with the potential to identify ST-elevation myocardial infarction (STEMI). If ECG is non-diagnostic, biomarkers such as Troponin can aid in the diagnosis. However, a negative initial biomarker does not rule out acute coronary syndrome (ACS). Additional testing such as stress testing or coronary angiography may be warranted based on risk stratification.
Management of ACP is guided by the likely etiology. For suspected ACS, early reperfusion therapy is crucial. High-risk patients may benefit from early invasive strategies. Conversely, low-risk patients may be managed conservatively. Non-cardiac causes of ACP should also be considered and managed accordingly.
Clinical Decision Units (CDUs) have emerged as a valuable resource for managing patients with ACP. They provide a setting for continued observation, further testing, and decision-making. CDUs have been shown to reduce hospital admissions and healthcare costs without compromising patient outcomes.
Diagnosing and managing ACP requires a systematic approach, combining clinical assessment, risk stratification, diagnostic testing, and appropriate management strategies. The use of CDUs can optimize this process, improving patient outcomes while reducing healthcare costs. As our understanding of ACP evolves, so too must our approach to its evaluation and management.
