Acute cardiovascular collapse during pregnancy is a rare but catastrophic event, posing significant risks to both maternal and fetal health. This review synthesizes current scientific evidence, clinical observations, and guideline-based recommendations to highlight the epidemiology, pathophysiology, risk factors, clinical presentation, diagnostic strategies, management protocols, and evolving therapies for acute cardiovascular collapse in pregnant patients. Insights are provided to facilitate timely recognition, evidence-based intervention, and optimized outcomes in this vulnerable population.
Pregnancy induces profound physiological changes in the cardiovascular system, rendering pregnant individuals susceptible to unique forms of hemodynamic compromise. Acute cardiovascular collapse characterized by sudden loss of effective circulation and tissue perfusion remains a rare emergency but is associated with extremely high morbidity and mortality. Early recognition and rapid, coordinated multidisciplinary response are crucial for maternal and fetal survival. This article aims to provide a comprehensive overview of acute cardiovascular collapse during pregnancy, integrating recent evidence and clinical guidelines to inform best practices for healthcare professionals.
The incidence of acute cardiovascular collapse in pregnancy is estimated at approximately 1 in 20,000 deliveries, with variation depending on population characteristics and healthcare resources. Despite its rarity, the event accounts for a disproportionate number of maternal deaths worldwide. Major causes include amniotic fluid embolism, massive obstetric hemorrhage, pulmonary embolism, anaphylaxis, and cardiac arrhythmias. Maternal mortality rates can approach 60%, and perinatal outcomes are equally grave, particularly if collapse occurs remote from immediate medical care. Epidemiological trends underscore the importance of heightened vigilance in high-risk populations and healthcare settings.
Acute cardiovascular collapse in pregnancy may result from a spectrum of mechanisms. Amniotic fluid embolism is characterized by abrupt entry of amniotic components into the maternal circulation, provoking an inflammatory and coagulopathic cascade. Massive hemorrhage leads to hypovolemic shock via rapid loss of intravascular volume. Cardiac arrhythmias exacerbated by electrolyte imbalances or underlying structural heart disease can precipitate sudden loss of cardiac output. Additionally, pregnancy-associated physiological adaptations, including increased blood volume, cardiac output, and decreased systemic vascular resistance, may mask early signs of decompensation, thereby delaying recognition until advanced collapse ensues.

In the setting of amniotic fluid embolism, coagulopathy and disseminated intravascular coagulation may rapidly ensue, complicating resuscitative efforts. Differential diagnosis encompasses massive pulmonary embolism, myocardial infarction, arrhythmia, anaphylaxis, and eclampsia. A high index of suspicion and rapid assessment are paramount to minimizing delays in intervention.
Diagnosis of acute cardiovascular collapse during pregnancy is predominantly clinical, predicated on recognition of circulatory arrest or shock in the peripartum setting. Immediate assessment includes evaluation of airway, breathing, and circulation, with concurrent monitoring of vital parameters and fetal status. Electrocardiography, echocardiography, and focused ultrasonography may aid in identifying reversible causes such as pericardial tamponade or massive pulmonary embolism. Laboratory studies, including arterial blood gases, coagulation profile, and cardiac biomarkers, provide adjunctive information but should not delay initiation of life-saving interventions.
Management of acute cardiovascular collapse in pregnancy necessitates prompt, systematic resuscitation following advanced cardiac life support (ACLS) principles, with modifications specific to the pregnant patient. Left uterine displacement is critical to relieve aortocaval compression and optimize venous return. Early intubation and mechanical ventilation may be required to secure the airway and support oxygenation. Intravenous access should be established for rapid volume resuscitation with crystalloids and blood products as indicated. Specific etiological treatments such as administration of tranexamic acid for hemorrhage or emergent perimortem cesarean delivery if maternal resuscitation is unsuccessful within four minutes are vital to improving both maternal and fetal outcomes. Multidisciplinary coordination among obstetricians, anesthesiologists, intensivists, and neonatologists is essential throughout resuscitative efforts.
Recent advances have focused on refined protocols for maternal cardiac arrest, including updated ACLS algorithms that emphasize the importance of early perimortem cesarean section and team-based simulation training. Point-of-care ultrasonography has emerged as a valuable tool for rapid differentiation of causes of collapse, expediting targeted management. The use of advanced hemodynamic monitoring and extracorporeal membrane oxygenation (ECMO) has been reported in select cases, offering potential salvage therapy for refractory cardiac arrest. Ongoing research into the pathogenesis of amniotic fluid embolism and improved blood product resuscitation strategies continues to shape future standards of care.
Current guidelines from the American Heart Association and the Royal College of Obstetricians and Gynaecologists advocate for immediate, high-quality basic and advanced life support interventions, with specific recommendations for manual left uterine displacement and early consideration of perimortem cesarean delivery. Algorithm-based team response, anticipation of massive transfusion, and utilization of simulation-based training are strongly encouraged. Institutional protocols should be established and regularly reviewed to ensure rapid mobilization of resources and personnel in the event of maternal cardiovascular collapse.
Acute cardiovascular collapse during pregnancy remains a formidable clinical challenge, demanding rapid recognition, expert multidisciplinary intervention, and adherence to evidence-based protocols to optimize maternal and fetal survival. Advances in diagnostic modalities, resuscitation strategies, and guideline-based practices have improved outcomes, yet continued vigilance and ongoing research are necessary to further reduce morbidity and mortality. Healthcare professionals must remain updated on evolving evidence and maintain readiness to respond effectively to this rare but life-threatening obstetric emergency.
