Equitable access to fertility preservation is a critical aspect of reproductive health, particularly for individuals at risk of infertility due to medical interventions or underlying conditions. Recent public health policies aim to mitigate disparities in access, integrating evidence-based strategies, insurance reforms, and educational initiatives. This review synthesizes current epidemiological data, clinical considerations, mechanisms underlying iatrogenic infertility, and the impact of public health policies on improving equity in fertility preservation services. The discussion underscores ongoing challenges and future directions for policy optimization, clinical practice, and patient advocacy.
Fertility preservation encompasses a range of medical interventions intended to safeguard reproductive potential in individuals facing imminent gonadotoxic therapies or other fertility-threatening conditions. As cancer survival rates improve and awareness of non-malignant causes of infertility grows, demand for fertility preservation has surged. However, significant disparities persist in access to these services, driven by socioeconomic, racial, geographic, and systemic barriers. Public health policies play a pivotal role in addressing these inequities, necessitating a thorough understanding of the epidemiology, clinical challenges, and policy landscape to guide effective advocacy and evidence-based interventions.
Infertility affects an estimated 8–12% of couples globally, with iatrogenic infertility representing a growing subset due to advances in cancer therapies, hematopoietic stem cell transplantation, and autoimmune disease treatments. In the United States alone, approximately 70,000 reproductive-age cancer patients are diagnosed annually, with up to 80% receiving gonadotoxic therapy. Disparities in access to fertility preservation are exacerbated among minority populations, those with lower socioeconomic status, rural residents, and patients lacking comprehensive insurance coverage. Studies demonstrate that less than 50% of eligible oncology patients are referred for fertility preservation, and actual utilization rates are even lower, underscoring the need for systematic public health interventions.
Gonadotoxic agents, including alkylating chemotherapeutics, radiation, and certain immunosuppressants, cause direct damage to ovarian follicles and testicular germ cells, resulting in diminished or absent gametogenesis. Mechanistically, DNA cross-linking, oxidative stress, and microvascular injury impair reproductive tissue, leading to premature ovarian insufficiency or azoospermia. The extent of damage is influenced by dose, agent type, patient age, baseline gonadal reserve, and underlying comorbidities. Understanding these mechanisms informs risk stratification and individualized fertility preservation planning.
Key risk factors for iatrogenic infertility include younger age at treatment (due to heightened sensitivity of developing gonads), type and cumulative dose of chemotherapeutic agents, pelvic or total body irradiation, and certain surgical interventions (e.g., oophorectomy, orchiectomy). Genetic predispositions, such as BRCA mutations, may also elevate baseline infertility risk. Socioeconomic factors, limited health literacy, and lack of provider awareness further compound risk by impeding timely referral and intervention.
Clinical indicators of impending or established infertility vary by sex and underlying etiology. In females, amenorrhea, oligomenorrhea, and elevated gonadotropin levels signal ovarian insufficiency, while males may present with oligospermia, azoospermia, or testicular atrophy. In many cases, the window for effective fertility preservation is narrow, necessitating prompt identification and intervention. Clinicians must maintain a high index of suspicion and routinely discuss reproductive risks with all at-risk patients prior to initiating gonadotoxic therapy.
Assessment of fertility potential relies on a combination of clinical history, hormonal evaluation (FSH, LH, estradiol, anti-Müllerian hormone in females; FSH, LH, testosterone in males), and imaging studies (antral follicle count, testicular ultrasound). Semen analysis remains the cornerstone for evaluating male fertility. Timely diagnosis of diminished reserve is essential to guide counseling and intervention, and should be integrated into pre-treatment workups for all patients at risk of iatrogenic infertility.
Fertility preservation options include oocyte and embryo cryopreservation, ovarian tissue cryopreservation, sperm banking, and testicular tissue freezing. The choice of modality is dictated by patient age, time constraints, underlying pathology, and personal preferences. Multidisciplinary collaboration among oncologists, reproductive endocrinologists, and mental health professionals is crucial to optimizing outcomes. Public health policies increasingly advocate for standardized referral pathways, patient education, and financial support mechanisms to facilitate access to these services.
Recent years have witnessed significant advances in fertility preservation, including improved cryopreservation techniques (vitrification), in vitro maturation of oocytes, and experimental strategies such as artificial ovary development and stem cell–based gametogenesis. Expanded indications, such as fertility preservation for gender-diverse individuals and those with non-oncologic chronic illnesses, reflect growing recognition of reproductive autonomy. Policy reforms in several jurisdictions now mandate insurance coverage for fertility preservation in medically indicated cases, a trend supported by accumulating evidence of cost-effectiveness and psychosocial benefit.
Professional societies, including ASCO, ESHRE, and ASRM, recommend that all patients at risk of treatment-related infertility receive timely counseling on fertility preservation options, ideally prior to initiation of gonadotoxic therapy. Guidelines emphasize multidisciplinary coordination, culturally sensitive communication, and the elimination of financial and systemic barriers. Public health agencies are urged to incorporate fertility preservation into comprehensive cancer care and chronic disease management, with ongoing surveillance to monitor equity and access outcomes.
The imperative for equitable access to fertility preservation services is increasingly recognized in public health policy and clinical practice. Persistent disparities in referral, utilization, and insurance coverage highlight the need for targeted interventions, robust policy frameworks, and continued advocacy. Recent advances in medical technology and expanding guideline recommendations provide new opportunities to improve reproductive autonomy and quality of life for diverse patient populations. Ongoing research, education, and policy innovation will be essential to achieving truly equitable access to fertility preservation worldwide.
