Optimizing endometrial health prior to assisted reproductive technologies (ART) is pivotal for improving implantation rates and overall reproductive outcomes. This review synthesizes current clinical guidelines, recent advances, and practical strategies for managing endometrial factors that influence ART success. A detailed evidence-based approach is provided, encompassing epidemiology, pathophysiology, risk factors, diagnostic modalities, management options, and guideline-driven interventions. The article emphasizes the importance of individualized patient assessment, highlights emerging therapies, and offers actionable recommendations for clinicians managing women undergoing ART.
Assisted reproduction has revolutionized infertility management, yet successful outcomes remain dependent on multifactorial determinants, with endometrial receptivity being a critical factor. The endometrium, as the site of embryo implantation, requires a precisely synchronized, hormonally primed environment to facilitate successful pregnancy. Suboptimal endometrial conditions are associated with decreased implantation, increased miscarriage risk, and lower live birth rates. Thus, pre-ART endometrial optimization is a focal point in reproductive medicine, warranting thorough evaluation, evidence-driven intervention, and adherence to contemporary guidelines.
Infertility affects an estimated 8-12% of reproductive-aged couples worldwide, with endometrial factors implicated in 2-3% of cases. However, subclinical endometrial dysfunction may contribute to higher proportions of unexplained infertility and repeated implantation failure (RIF) after ART. Epidemiological studies underscore the prevalence of endometrial pathologies such as polyps (up to 24% in infertile women), chronic endometritis (10-30%), and suboptimal endometrial thickness, reinforcing the need for systematic evaluation prior to ART cycles.
Endometrial receptivity is governed by a tightly regulated interplay of hormonal, immune, vascular, and molecular signals. Key events include estrogen-mediated proliferation, progesterone-driven differentiation, and the establishment of a receptive window, characterized by pinopode formation, integrin expression, and cytokine milieu conducive to embryo attachment. Disruption of these processes due to inflammation, infection, inadequate vascularization, or aberrant hormonal signaling can compromise endometrial readiness, impairing implantation and pregnancy maintenance.
Identified risk factors for endometrial dysfunction include advanced maternal age, obesity, polycystic ovary syndrome (PCOS), uterine malformations, prior uterine surgery, intrauterine adhesions (Asherman syndrome), chronic endometritis, and exposure to endocrine disruptors. Repeated ART failure, recurrent pregnancy loss, and history of unresponsive endometrial lining during stimulation cycles also warrant heightened clinical suspicion and targeted workup.
Endometrial pathologies may be asymptomatic or manifest as abnormal uterine bleeding, pelvic pain, or infertility. Chronic endometritis can be subtle, presenting with mild discharge or intermenstrual spotting. Ultrasound findings such as thin endometrium (<7 mm), irregular endometrial lining, or intracavitary lesions frequently prompt further investigation. Clinical features should be correlated with cycle history and previous ART outcomes for comprehensive assessment.
A thorough diagnostic approach integrates transvaginal ultrasonography, saline infusion sonohysterography, hysteroscopy, and targeted endometrial biopsy. Ultrasound evaluates endometrial thickness, pattern, and structural anomalies. Hysteroscopy remains the gold standard for detecting intrauterine pathology (e.g., adhesions, polyps, submucosal fibroids) and allows direct visualization with the possibility for therapeutic intervention. Histopathological evaluation identifies chronic endometritis (plasma cell infiltration), hyperplasia, or neoplastic changes. Emerging molecular assays, such as the endometrial receptivity array (ERA), assess transcriptomic signatures to personalize embryo transfer timing.
Management strategies are tailored to underlying pathology. Surgical removal of intrauterine lesions (hysteroscopic resection of polyps, fibroids, or adhesions) improves endometrial architecture and receptivity. Antibiotic therapy targets chronic endometritis, with doxycycline-based regimens commonly used. Hormonal manipulation optimizes endometrial development estrogen supplementation for inadequate proliferation, and progesterone support to ensure luteal phase sufficiency. Adjunctive therapies include low-dose aspirin, pentoxifylline, granulocyte colony-stimulating factor (G-CSF), and intrauterine platelet-rich plasma, though evidence remains evolving. Strict glycemic and weight control, smoking cessation, and correction of thyroid dysfunction further support endometrial health.
Recent research has focused on regenerative and precision medicine approaches. Stem cell-based therapies, autologous platelet-rich plasma, and intrauterine infusion of growth factors aim to restore endometrial function in refractory cases. Molecular profiling (e.g., ERA) enables personalized embryo transfer, potentially enhancing implantation rates. The role of the microbiome is being elucidated, with probiotic or antibiotic modulation under investigation. Immunomodulatory therapies, such as intrauterine hCG, tacrolimus, and endometrial scratching, are studied for their capacity to augment receptivity, though robust evidence is pending.
Society guidelines including those from the American Society for Reproductive Medicine (ASRM) and the European Society of Human Reproduction and Embryology (ESHRE) underscore individualized assessment and evidence-based intervention. Routine pre-ART hysteroscopy is advised in women with risk factors or prior ART failure. Surgical correction of identified pathology is recommended. Empirical endometrial scratching is not routinely endorsed outside of research settings. The use of adjuvant therapies is reserved for select indications, with clinicians encouraged to balance potential benefits against known risks. Ongoing research is supported to refine predictive markers and optimize therapeutic strategies.
Optimizing endometrial health is a cornerstone of successful ART outcomes. Current guidelines advocate for comprehensive evaluation, correction of modifiable factors, and judicious use of emerging therapies. Integrating recent advances with established clinical protocols enables tailored, patient-centric care. Continued research into endometrial biology and targeted interventions holds promise for further enhancing reproductive success for women undergoing assisted reproduction.
