Family practice occupies a unique position in healthcare, offering continuous, comprehensive care to individuals across the lifespan. The concept of life course wellness planning recognizes that health trajectories are shaped by a complex interplay of biological, behavioral, and social factors, beginning before birth and continuing into old age. This approach shifts the focus from episodic illness management to anticipatory guidance and personalized prevention, aiming to reduce the cumulative burden of chronic disease. Family practitioners are increasingly called upon to implement structured wellness strategies that adapt to patients’ evolving needs, leveraging evidence-based interventions at critical windows of opportunity.
Chronic diseases, including cardiovascular disease, diabetes, and certain cancers, account for the majority of morbidity and mortality globally.
