Abstract

Chronic liver dysfunction is a complex condition that significantly impairs patients quality of life (QoL) due to a spectrum of physical, psychological, and social burdens. This review synthesizes current evidence on strategies for maintaining daily activities as a means to enhance QoL in patients with chronic liver disease (CLD), integrating recent clinical guidelines, mechanistic insights, and practical approaches. Emphasizing a multidisciplinary strategy, the article highlights functional assessment, risk stratification, and tailored interventions that support daily functioning and mitigate the adverse effects of chronic liver impairment. The clinical implications for healthcare providers are discussed, with recommendations for optimizing patient outcomes through evidence-based, patient-centered care.

Introduction

Chronic liver dysfunction encompasses a spectrum of diseases, including cirrhosis, chronic hepatitis, and non-alcoholic fatty liver disease, characterized by progressive hepatic impairment and multisystem involvement. The resulting decline in physical capacity, cognitive function, and psychosocial wellbeing often leads to compromised daily activities and diminished QoL. Given the chronicity of these conditions, maintaining engagement in daily activities is vital for preserving autonomy, preventing functional decline, and improving clinical outcomes. This review aims to provide clinicians with a comprehensive, evidence-based approach to supporting daily function in CLD, with a focus on mechanistic understanding, risk assessment, and practical interventions.

Epidemiology / Disease Burden

Chronic liver disease is a major global health challenge, with an estimated prevalence exceeding 1.5 billion individuals worldwide. Leading etiologies include chronic viral hepatitis (HBV, HCV), alcohol-related liver disease, and non-alcoholic steatohepatitis (NASH), with rising incidence of metabolic-associated fatty liver disease (MAFLD) paralleling the obesity epidemic. Functional impairment in chronic liver dysfunction manifests as reduced exercise tolerance, muscle weakness, impaired balance, cognitive slowing, and difficulties with self-care and instrumental activities. Patients frequently report chronic fatigue, impaired concentration, decreased motivation, and loss of independence. Treatment & Management

Optimizing daily activities in CLD necessitates an integrative strategy targeting both hepatic and extrahepatic factors. Nutritional support, with emphasis on adequate protein intake and correction of micronutrient deficiencies, underpins muscle preservation and physical function. Structured exercise programs, including resistance and aerobic training, have demonstrated efficacy in improving muscle mass, strength, and endurance, translating to enhanced functional capacity and QoL. Management of hepatic encephalopathy, anemia, and other reversible complications is paramount. Psychosocial support and patient education should address mood, motivation, and adaptation to chronic illness, fostering engagement in meaningful activities. Interventions must be tailored to individual capability, with regular reassessment and goal setting.

Recent Advances / Emerging Therapies

Recent advances in the management of CLD-related functional impairment include novel pharmacologic agents (e.g., ammonia-lowering therapies, myostatin inhibitors), digital health platforms for remote monitoring, and tele-rehabilitation programs. Randomized trials have highlighted the potential of supervised exercise and multidisciplinary day hospitals in reducing frailty and improving ADL performance. Ongoing research into gut-liver-muscle axis modulation and anti-inflammatory interventions may yield new therapeutic avenues. Integration of wearable technologies and patient-reported outcomes into routine care is enhancing individualized monitoring and early intervention.

Major international guidelines emphasize routine assessment of frailty and functional status in all patients with CLD, with early intervention for those at risk. The European Association for the Study of the Liver (EASL) and American Association for the Study of Liver Diseases (AASLD) advocate for comprehensive nutritional evaluation, structured exercise programs, and multidisciplinary care models. Individualized rehabilitation plans, incorporating patient preferences and comorbidities, are recommended to maximize engagement and functional gains. Ongoing collaboration between hepatology, geriatrics, nutrition, physical therapy, and mental health services is crucial for optimal outcomes.

Conclusion

Maintaining daily activities is central to improving quality of life in patients with chronic liver dysfunction, with strong evidence supporting integrated, patient-centered approaches. Early identification of functional decline, targeted nutritional and physical interventions, and multidisciplinary coordination are key to preserving independence and optimizing clinical outcomes. As the burden of chronic liver disease continues to rise globally, prioritizing functional assessment and supportive care must remain at the forefront of clinical practice and research.
