Understanding the epidemiology and risk factors is crucial in identifying patients at risk and tailoring the evaluation process.
The clinical evaluation of thyroid nodules includes a thorough history and physical examination. Symptoms such as rapid growth, hoarseness, dysphagia, or cervical lymphadenopathy may suggest malignancy. On examination, hard, fixed, or irregular nodules are more concerning for malignancy.
Diagnostic modalities for thyroid nodules include ultrasound, fine-needle aspiration (FNA), and molecular testing. Ultrasound characteristics such as hypoechogenicity, microcalcifications, irregular margins, taller-than-wide shape, and increased vascularity can suggest malignancy. FNA is the gold standard for cytologic evaluation, and molecular testing can provide additional information in indeterminate cases.
The management of thyroid nodules depends on the risk of malignancy. Low-risk nodules can be observed, while high-risk nodules may require surgery. Decisions should be individualized, taking into account the patient's age, comorbidities, and preferences.
