In vitro fertilization (IVF) has witnessed significant technological and clinical progress over recent years, dramatically improving outcomes for individuals and couples experiencing infertility. This review synthesizes the latest evidence on epidemiology, mechanisms, clinical features, diagnostic approaches, traditional and innovative management strategies, and emerging therapies in IVF. It incorporates guideline-based recommendations and recent research insights to offer a comprehensive resource for healthcare professionals engaged in the management of infertility.
Infertility remains a critical public health issue, affecting an estimated 8-12% of reproductive-aged couples globally. In vitro fertilization (IVF), first introduced in 1978, has become a cornerstone in assisted reproductive technologies (ART), with ongoing innovations reshaping clinical practice. This article provides an in-depth exploration of recent advances in IVF, focusing on evidence-based and mechanistic insights relevant to contemporary clinical practice.
Infertility affects approximately 186 million people worldwide, with variations in prevalence attributed to geographic, socioeconomic, and demographic factors. The use of IVF has increased, with over 2.5 million cycles performed annually and over 8 million births resulting from ART globally. The growing trend reflects both increased accessibility and heightened awareness, yet significant disparities persist in utilization, particularly in low-resource settings.
IVF is indicated in various pathophysiological contexts, including tubal factor infertility, male factor infertility, ovulatory disorders, unexplained infertility, and diminished ovarian reserve. The underlying mechanisms often involve disruptions in gametogenesis, fertilization, or embryo implantation. Advances in genomics and proteomics have elucidated molecular pathways central to folliculogenesis, oocyte maturation, and endometrial receptivity, guiding personalized approaches to IVF protocols.
Key risk factors for infertility include advanced maternal age, polycystic ovary syndrome (PCOS), endometriosis, pelvic inflammatory disease, genetic abnormalities, lifestyle factors (such as obesity, smoking, and excessive alcohol use), and environmental exposures. Male infertility may arise from oligospermia, asthenozoospermia, varicocele, or obstructive pathologies. Recognition of modifiable and non-modifiable risk factors allows for targeted interventions and counseling in IVF candidates.
Clinically, infertility presents as the inability to achieve pregnancy after 12 months of unprotected intercourse (or 6 months in women over 35). Associated symptoms may include menstrual irregularities, pelvic pain, or sexual dysfunction. Male partners may report erectile or ejaculatory difficulties. A thorough clinical assessment is essential to guide individualized diagnostic and treatment pathways.
Diagnostic evaluation encompasses a comprehensive history, physical examination, and targeted investigations. Female assessment involves hormonal profiling, ovarian reserve testing (AMH, antral follicle count), tubal patency studies (HSG, HyCoSy), and pelvic imaging. Male evaluation includes semen analysis, endocrine studies, and scrotal ultrasonography. Pre-IVF screening for infectious diseases and genetic counseling are integral to optimizing outcomes and minimizing risks.
Traditional IVF protocols include controlled ovarian hyperstimulation (COH) using gonadotropins, oocyte retrieval, in vitro fertilization, embryo culture, and embryo transfer. Luteal phase support, pre-implantation genetic testing (PGT), and cryopreservation techniques are routinely employed. Management is tailored based on patient age, ovarian reserve, underlying etiology, and previous ART response, with a multidisciplinary approach ensuring holistic care.
The past decade has seen transformative advances in IVF. Key innovations include time-lapse embryo imaging, which enhances embryo selection by enabling continuous morphokinetic assessment; non-invasive preimplantation genetic testing (niPGT) using cell-free DNA from spent culture media; and artificial intelligence (AI)-assisted algorithms for predicting embryo viability. The introduction of mild stimulation protocols and freeze-all strategies has reduced ovarian hyperstimulation syndrome (OHSS) risk and improved cumulative live birth rates. Mitochondrial replacement techniques offer hope for women with mitochondrial DNA disorders, while personalized medicine approaches—such as pharmacogenomics-guided ovarian stimulation and molecular profiling of endometrial receptivity (ERA)—have shown promise in optimizing success rates. Adjuncts like platelet-rich plasma (PRP) therapy for endometrial rejuvenation and stem cell therapies are under investigation. These advances are supported by robust clinical trials and meta-analyses, though further research is needed to validate long-term safety and efficacy.
International guidelines, including those from the ESHRE, ASRM, and NICE, emphasize individualized care, evidence-based stimulation protocols, single embryo transfer to minimize multiple pregnancies, and routine use of cryopreservation techniques. Updated recommendations advocate for comprehensive counseling, judicious use of add-ons, and integration of novel technologies only after demonstrating clear clinical benefit. The importance of psychosocial support, cost-effectiveness, and equitable access is increasingly recognized in contemporary practice.
Recent advances in IVF have profoundly impacted the management of infertility, offering improved success rates, enhanced patient safety, and greater personalization of care. The integration of novel technologies, molecular diagnostics, and precision medicine approaches is reshaping reproductive medicine. Ongoing research, multidisciplinary collaboration, and adherence to evidence-based guidelines are essential for continued progress and optimization of outcomes in IVF.
