Rehabilitation following recovery from high-containment infectious diseases (HCID) such as Ebola virus disease, Marburg, and Lassa fever presents unique challenges for healthcare professionals. Survivors often face complex multisystem sequelae including physical, neurological, psychological, and social impairments. This review synthesizes contemporary evidence, discusses the epidemiology, pathophysiology, and clinical manifestations relevant to post-HCID rehabilitation, and outlines practical, guideline-based approaches for optimizing long-term outcomes. Emphasis is placed on individualized, multidisciplinary care and the integration of recent advances for comprehensive recovery.
High-containment infectious diseases (HCIDs) are characterized by their propensity for nosocomial transmission, high mortality rates, and the need for specialized infection control measures. While increasing numbers of survivors are being discharged due to improved critical care and outbreak containment, their post-acute rehabilitation needs are complex and frequently under-recognized. Effective rehabilitation is vital not only for individual patient recovery but also for public health efforts to reduce disability and facilitate reintegration into society. This article explores the pathogenesis of sequelae, key clinical considerations, and evidence-based strategies for rehabilitation following HCID recovery, with a particular focus on the clinical and operational realities faced by healthcare professionals.
HCIDs, including viral hemorrhagic fevers (VHFs) such as Ebola and Marburg, as well as novel pathogens like SARS-CoV-2, have led to significant outbreaks globally over the past two decades. Survivorship has increased due to advances in supportive care, yet the burden of long-term sequelae remains substantial. Post-Ebola syndrome, for example, affects up to 70% of survivors, manifesting as chronic pain, neurocognitive deficits, and psychiatric morbidity. The burden is particularly acute in resource-limited settings, where access to rehabilitation services is sparse and stigma may impede care-seeking. Recent data highlight the need for scalable, context-sensitive rehabilitation frameworks to address these public health challenges.
The pathophysiological substrate of post-HCID morbidity is multifactorial. Direct viral cytopathic effects, immune-mediated injury, microvascular damage, and critical illness-related myopathy/neuroathy all contribute to persistent impairment. For VHFs, viral persistence in immune-privileged sites (e.g., CNS, ocular tissues) has been implicated in ongoing symptoms and delayed complications. Systemic inflammation, endothelial dysfunction, and the sequelae of prolonged immobilization further compound rehabilitation challenges. Understanding these mechanisms is essential for tailoring interventions and anticipating potential complications during recovery.
Risk factors for post-HCID sequelae and protracted rehabilitation include the severity of acute illness, duration of critical care, pre-existing comorbidities (such as diabetes or cardiovascular disease), older age, and delays in definitive care. Psychosocial determinants such as social isolation, stigma, and economic hardship may exacerbate disability and hinder rehabilitation engagement. Children and pregnant women may exhibit unique vulnerability to specific sequelae, necessitating tailored assessment and intervention strategies.
Survivors of HCIDs commonly experience a constellation of persistent symptoms. Musculoskeletal pain, profound fatigue, arthralgias, and muscle weakness are prevalent. Neurocognitive impairment, including memory deficits and concentration difficulties, has been documented across multiple HCIDs. Ocular complications (such as uveitis and vision loss) and sensorineural hearing loss are significant in certain viral syndromes. Psychological and psychiatric sequelae, including depression, anxiety, and post-traumatic stress disorder, are frequently reported and may be as debilitating as physical impairments. Social reintegration difficulties and stigma compound the clinical burden.
Comprehensive post-discharge assessment is essential for identifying the full spectrum of rehabilitation needs. A multidisciplinary approach is recommended, encompassing detailed history, functional assessment, neurocognitive evaluation, and targeted investigations (e.g., ophthalmic examination, audiometry, laboratory tests for organ function). Screening tools such as the Post-Ebola Syndrome Checklist or equivalent instruments may facilitate structured evaluation. Early identification of complications such as persistent viral reservoirs, secondary infections, or organ dysfunction is critical to inform safe and effective rehabilitation planning.
Rehabilitation interventions should be individualized and initiated as early as clinically feasible. Physical therapy focuses on restoring strength, endurance, and mobility, often utilizing graded exercise and functional retraining. Occupational therapy addresses activities of daily living (ADLs) and cognitive rehabilitation. Psychological support, including trauma-informed counseling and pharmacotherapy when indicated, is central to holistic care. Ocular and audiological interventions are important for those with sensory deficits. Coordination with infectious disease specialists is necessary to monitor for late complications and ensure infection control precautions are maintained as needed. Social work support may address barriers to access, stigma, and reintegration. Community-based rehabilitation and telehealth models have emerged as promising strategies in resource-constrained settings.
Recent advances in rehabilitation science have informed new approaches for HCID survivors. Tele-rehabilitation platforms have demonstrated feasibility and acceptability, especially where in-person services are limited or stigma is a barrier. Novel neurocognitive training modules, virtual reality-based physical therapy, and group-based psychosocial interventions are under investigation. Biomarker-driven risk stratification may enable targeted interventions for high-risk individuals. Integration of peer support programs has shown promise in improving engagement and outcomes. Research into antiviral therapies and immunomodulators continues to shape the landscape of long-term care by potentially reducing the burden of persistent viral reservoirs and immune-mediated damage.
International bodies such as the World Health Organization (WHO) and Centers for Disease Control and Prevention (CDC) have issued guidance on post-HCID care, emphasizing the need for early, multidisciplinary rehabilitation. Key recommendations include structured assessment of physical, cognitive, and psychological domains; patient and family education; regular follow-up; and community engagement to reduce stigma. Infection prevention and control (IPC) measures must be maintained until the risk of transmission is excluded. Access to rehabilitation should be prioritized as an integral component of outbreak response and survivor care packages. Adaptation of guidelines to local contexts is strongly encouraged.
Rehabilitation after high-containment infectious disease recovery is a critical, yet often underappreciated, component of comprehensive care. Survivors face a complex interplay of physical, neurocognitive, and psychosocial challenges that require individualized, multidisciplinary management. Recent advances in rehabilitation science and telehealth offer new opportunities for optimizing outcomes, particularly in resource-limited settings. Ongoing research and the development of context-specific guidelines are essential to ensure that all survivors can achieve maximal functional recovery and reintegrate successfully into their communities.
