Understanding the mechanisms, clinical implications, and management strategies of fever in pregnancy is essential for optimizing care and reducing morbidity. Recent studies underscore the need for a multidisciplinary approach that integrates obstetric, infectious disease, and neonatal expertise.

Epidemiology / Disease Burden

Fever is reported in approximately 10–15% of pregnancies, with higher rates observed in certain regions due to endemic infectious diseases. The etiology of febrile illness varies by gestational age, region, and underlying maternal health status. Common causes include urinary tract infections, chorioamnionitis, influenza, and other viral infections. Febrile morbidity and associated complications such as preterm labor, fetal loss, and congenital anomalies underscore the public health importance of effective fever management during pregnancy.

Pathophysiology

Diagnosis

Diagnostic evaluation of fever in pregnancy should be rapid and thorough, aiming to identify the underlying etiology and assess fetal well-being. Investigations include complete blood count, urinalysis and urine culture, blood cultures, chest radiography (with abdominal shielding), and targeted serologies or PCR assays based on clinical suspicion. Ultrasonography is valuable for detecting intrauterine infection or fetal distress. Amniocentesis may be indicated in select cases for microbiological assessment of amniotic fluid.

Treatment & Management

Management strategies should be individualized based on gestational age, severity of illness, and suspected etiology. Empiric antibiotic therapy is warranted in cases of suspected bacterial infection, with regimen selection guided by local resistance patterns and safety profiles during pregnancy. Supportive care includes antipyretics (preferably acetaminophen), intravenous fluids, and close fetal monitoring. In severe cases, hospitalization and multidisciplinary management are indicated. Maternal hyperthermia should be promptly treated to reduce the risk of adverse fetal outcomes.

Recent Advances / Emerging Therapies

Recent advances in diagnostic technology, such as rapid molecular assays and point-of-care testing, have improved the speed and accuracy of pathogen identification. Novel therapeutic approaches, including targeted immunomodulatory therapies and next-generation antimicrobials, are being explored, particularly for resistant and atypical pathogens. There is growing interest in the role of maternal vaccination (e.g., influenza, COVID-19) in reducing the incidence and severity of febrile illness during pregnancy. Telemedicine platforms are increasingly used for triage and follow-up, improving access to care in underserved populations.

Guideline Recommendations

Current guidelines from organizations such as the American College of Obstetricians and Gynecologists (ACOG) and the Centers for Disease Control and Prevention (CDC) emphasize the importance of early recognition and treatment of fever in pregnancy. Key recommendations include prompt initiation of empiric therapy for suspected serious infection, avoidance of contraindicated medications (e.g., NSAIDs in the third trimester), and routine vaccination as part of prenatal care. Fetal monitoring and consideration of delivery in cases of refractory maternal sepsis are also highlighted. Multidisciplinary collaboration is essential to optimize outcomes.
