The digital era has ushered in a paradigm shift in the diagnosis, treatment, and management of otolaryngological (ENT) disorders. This review explores how technological advancements, such as telemedicine, artificial intelligence (AI), digital imaging, and novel therapeutic modalities, are revolutionizing ENT practice. We synthesize current evidence, highlight guideline-based recommendations, and discuss the clinical relevance of these innovations for contemporary otolaryngology. Special emphasis is placed on the epidemiology and burden of ENT diseases, the underlying pathophysiological mechanisms, diagnostic techniques, and transformative digital solutions that optimize patient outcomes. The review provides a comprehensive perspective for clinicians aiming to integrate cutting-edge digital tools into everyday ENT care.
Otolaryngology, or ear, nose, and throat (ENT) medicine, encompasses a diverse array of diseases affecting millions globally. Historically, the specialty has been characterized by its reliance on clinical skills and visualization tools. However, with the rapid advancement of digital technologies, ENT is experiencing a profound transformation. From the adoption of electronic health records (EHRs) and telemedicine to the integration of AI-powered diagnostic tools and image-guided interventions, the digital revolution is enabling more precise, efficient, and patient-centered care. This review aims to synthesize current evidence on transformative digital solutions in ENT, offering clinicians an in-depth understanding of their mechanisms, benefits, and practical implications in daily practice.
ENT diseases constitute a significant proportion of the global disease burden, with conditions such as chronic rhinosinusitis, allergic rhinitis, otitis media, hearing loss, and head and neck cancers contributing to substantial morbidity and healthcare costs. According to the World Health Organization, over 430 million people worldwide require rehabilitation for disabling hearing loss, and upper respiratory tract infections remain among the most common reasons for primary care consultations. Digital innovations are particularly relevant in low-resource settings and rural areas, where access to specialized ENT care is limited. The growing prevalence of chronic ENT conditions and an aging population underscore the pressing need for scalable, efficient, and accessible solutions.
Understanding the pathophysiological mechanisms underlying ENT diseases is essential for targeted interventions. For instance, the pathogenesis of chronic rhinosinusitis involves an interplay of host immune responses, microbial dysbiosis, and environmental factors, while sensorineural hearing loss is often associated with cochlear hair cell degeneration secondary to genetic mutations, noise exposure, or ototoxic medications. Digital pathology and genomics are offering novel insights into the molecular and cellular basis of these conditions, facilitating early identification of at-risk individuals and personalized therapeutic approaches. Advanced digital stethoscopes, endoscopes, and imaging platforms are augmenting the clinician’s ability to detect subtle abnormalities and monitor disease progression. Accurate diagnosis in ENT relies on a combination of clinical assessment, endoscopic visualization, audiometric testing, and imaging studies. Digital advancements have revolutionized diagnostic workflows. High-resolution video-endoscopy, digital otoscopy, and AI-powered image analysis enable objective assessment and real-time decision support. Tele-ENT platforms facilitate remote consultations, allowing for expert review of digital images and audiovisual recordings, thereby bridging geographical gaps in specialist access. Integration of diagnostic data into EHRs enhances continuity of care and supports multidisciplinary management.
Management strategies in ENT now increasingly leverage digital tools for personalized therapy and improved adherence. For instance, hearing aids and cochlear implants are equipped with wireless connectivity, enabling real-time adjustments and remote troubleshooting. Digital health applications offer patient education, symptom tracking, and medication reminders, promoting self-management and reducing the burden on healthcare systems. Robotic-assisted surgery, image-guided navigation, and simulation-based training are improving surgical precision and outcomes in complex ENT procedures. Tele-rehabilitation platforms provide accessible speech, language, and swallowing therapy, enhancing recovery in post-operative and chronic ENT patients.
The past decade has seen exponential growth in emerging technologies for ENT practice. AI and machine learning algorithms are being deployed for automated detection of head and neck cancers on imaging, risk prediction in obstructive sleep apnea, and optimization of cochlear implant programming. 3D printing is facilitating patient-specific prosthetics and surgical planning. Digital biomarkers derived from wearable sensors and smartphone apps are enabling continuous monitoring of conditions such as sleep apnea and vertigo. Virtual reality (VR) and augmented reality (AR) platforms are transforming medical education and intraoperative visualization, enhancing surgeon training and patient safety.
Professional societies, including the American Academy of Otolaryngology–Head and Neck Surgery and the European Federation of Otorhinolaryngological Societies, endorse the integration of digital solutions in routine ENT care. Current guidelines highlight the use of telemedicine for triage and follow-up, AI-assisted diagnostics for early cancer detection, and electronic data capture for clinical audits and quality improvement. There is a growing emphasis on ensuring data privacy, cybersecurity, and equitable access to digital health technologies. Continuous professional development and training in digital competencies are recommended to empower clinicians to harness these transformative tools effectively.
The digital transformation in ENT is reshaping the landscape of clinical practice, research, and education. By harnessing the power of telemedicine, AI, digital imaging, and connected therapeutics, clinicians can deliver more efficient, accurate, and patient-centered care. Ongoing research, robust regulatory frameworks, and interdisciplinary collaboration are essential to fully realize the potential of these innovations while safeguarding ethical standards and equity.
