This review synthesizes current literature, evaluates the epidemiology and clinical burden associated with perioperative care, and discusses the mechanisms, risk factors, and practical implications of personalized optimization strategies. It also covers diagnostic and management frameworks, recent advances, and guideline-based recommendations, aiming to inform clinicians and healthcare professionals about evidence-based approaches to improve perioperative QoL outcomes.
The perioperative period, encompassing preoperative, intraoperative, and postoperative phases, is a critical window with profound implications for patient recovery, morbidity, and overall quality of life. Traditional perioperative management often employs standardized protocols, yet patients present with diverse physiological and psychosocial profiles. A shift toward personalized perioperative optimization aims to tailor interventions to individual needs, maximizing benefit and minimizing harm. This approach is rooted in robust clinical research linking tailored perioperative care to improved patient satisfaction, reduced complications, and enhanced functional outcomes. The global volume of surgical procedures exceeds 300 million annually, with significant perioperative morbidity and mortality contributing to healthcare burdens. Adverse perioperative events, including surgical site infections, thromboembolic phenomena, and postoperative delirium, are associated with poorer QoL outcomes and increased resource utilization. Studies reveal that up to 20% of surgical patients experience significant functional decline or diminished QoL within six months postoperatively, highlighting the need for interventions that not only prevent complications but also address holistic patient well-being. PROMs such as the EQ-5D and SF-36 enable quantification of patient-centered QoL domains, facilitating shared decision-making and individualized care planning. Recent advances include the use of digital health technologies and predictive analytics to enhance risk stratification and guide personalized interventions.
Personalized perioperative optimization integrates evidence-based interventions across the surgical continuum. Prehabilitation programs comprising exercise, nutritional, and psychological support have demonstrated efficacy in enhancing functional capacity and postoperative recovery.
