Chronic cough, defined as a cough that persists for more than eight weeks, often presents a diagnostic challenge to clinicians. The myriad potential etiologies and overlapping symptomatology can make the diagnosis and management of chronic cough complex. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
Initial evaluation of a patient with chronic cough should include a detailed history and physical examination to identify potential causes. The most common causes are upper airway cough syndrome (UACS), asthma, and gastroesophageal reflux disease (GERD). However, other causes such as chronic bronchitis, bronchiectasis, and lung cancer should also be considered. Diagnostic testing, including chest X-ray, pulmonary function tests, and bronchoscopy, can be helpful in identifying the underlying cause.
Treatment should be tailored to the underlying cause. For patients with UACS, antihistamines and nasal steroids may be beneficial. In cases of asthma, inhaled corticosteroids and bronchodilators are the mainstay of treatment. Proton pump inhibitors can be effective for cough due to GERD. For patients with no identifiable cause, a trial of empirical therapy may be considered. Multimodal therapy, including behavioral therapy, may also be beneficial in some patients.
A multidisciplinary approach involving pulmonologists, gastroenterologists, allergists, and otolaryngologists can be valuable in complex cases. This collaborative approach can facilitate comprehensive evaluation, enhance patient care, and improve outcomes.
Chronic cough is a complex condition that requires a thorough and systematic approach to diagnosis and management. By understanding the common causes, utilizing appropriate diagnostic tests, and implementing tailored treatment strategies, clinicians can effectively manage this challenging condition. A multidisciplinary approach can further enhance patient care and outcomes in complex cases.
