By mapping these hormonal transitions longitudinally, clinicians can better understand the epidemiology, pathophysiology, risk factors, clinical features, and management strategies for common endocrine-related health issues in women. The review emphasizes clinically relevant mechanisms, integrates recent research findings, discusses emerging therapies, and highlights guideline-based recommendations to optimize women's health outcomes across all life stages.

Introduction

Endocrine function in women is characterized by distinct phases that influence health across the lifespan. The interplay between hypothalamic-pituitary-gonadal (HPG) axis activity, ovarian hormones, adrenal steroids, and metabolic regulators underpins critical transitions such as puberty, reproductive maturity, perimenopause, and postmenopause. Understanding the temporal mapping of these hormonal changes is crucial for clinicians to anticipate, diagnose, and manage a spectrum of gynecologic, metabolic, neuroendocrine, and psychosocial conditions that are unique or more prevalent in women.

Epidemiology / Disease Burden

Globally, women experience a unique burden of endocrine-related diseases, many of which are closely linked to age-dependent hormonal changes. Conditions such as polycystic ovary syndrome (PCOS), endometriosis, infertility, menopausal syndrome, osteoporosis, and cardiovascular disease display distinct age-based prevalence patterns. In adulthood, menstrual irregularities, infertility, or abnormal uterine bleeding may signal underlying endocrine dysfunction. Recognition of these features in the context of hormonal transitions is key to timely diagnosis.

Assessment may include measurement of gonadotropins, estradiol, anti-Müllerian hormone (AMH), thyroid function, and metabolic parameters. Imaging (pelvic ultrasound, DXA scan) aids in evaluating ovarian and bone health. Age-appropriate screening (e.g., for osteoporosis or thyroid dysfunction postmenopause) is guided by established risk stratification algorithms. Recent advances in biomarker discovery and genomic testing are enhancing diagnostic precision in complex cases.

Treatment & Management

Management strategies are tailored to the life stage and specific endocrine disorder. Adolescent and reproductive-aged women with PCOS benefit from lifestyle modification, hormonal contraceptives, and insulin sensitizers. Fertility management may involve ovulation induction or assisted reproductive technologies. Perimenopausal and postmenopausal women with vasomotor symptoms may require individualized menopausal hormone therapy (MHT), balancing benefits against risks. Osteoporosis prevention involves calcium, vitamin D, weight-bearing exercise, and pharmacotherapy (bisphosphonates, SERMs) as indicated. Multidisciplinary care, including endocrinology, gynecology, and primary care, is essential for optimal outcomes.

Recent Advances / Emerging Therapies

Recent research has yielded novel insights into endocrine regulation and therapeutic options. Advances in AMH measurement provide improved assessment of ovarian reserve and reproductive lifespan prediction. Non-hormonal therapies for menopausal symptoms (e.g., selective neurokinin-3 receptor antagonists) are emerging as alternatives to MHT. In PCOS, new agents targeting metabolic dysfunction (incretin-based therapies, anti-androgens) are under investigation.
