Fever of unknown origin (FUO) remains a complex clinical challenge in adult medicine, demanding a systematic and evidence-based diagnostic approach. This article offers an in-depth review of the epidemiology, pathophysiology, risk factors, clinical features, diagnostic strategies, management principles, recent advances, and guideline-based recommendations for FUO in adults. Emphasis is placed on integrating current research findings and practical clinical insights to support healthcare professionals in the optimal evaluation and management of this multifaceted syndrome.
Fever of unknown origin, traditionally defined by Petersdorf and Beeson as a temperature exceeding 38.3°C (101°F) on several occasions for at least three weeks with no identified cause after one week of inpatient investigation, continues to represent a diagnostic dilemma. Despite advances in diagnostic modalities, the etiological spectrum remains broad and can be influenced by geographic, demographic, and temporal factors. A structured approach is essential to distinguish benign self-limiting fevers from potentially life-threatening conditions, reducing unnecessary interventions while ensuring timely diagnosis and management.
The incidence of FUO among hospitalized adults varies between 2% and 3%, with patterns shifting due to changing prevalence of infections, malignancies, and autoimmune disorders. Recent multicenter studies indicate that infectious etiologies still predominate in developing regions, whereas noninfectious inflammatory diseases and malignancies are increasingly prominent in developed countries. In approximately 10–20% of cases, no definitive diagnosis is reached despite extensive evaluation, underscoring the persistent clinical burden and resource utilization associated with FUO.
Fever results from the endogenous pyrogenic response to infectious or noninfectious stimuli, mediated primarily by cytokines such as interleukin-1, interleukin-6, tumor necrosis factor-alpha, and interferons. In FUO, the inability to localize or eradicate the trigger stems from atypical presentations, occult sites of infection or neoplasm, immune dysregulation, or rare diseases. The pathophysiological spectrum encompasses infections (e.g., tuberculosis, endocarditis), neoplasms (e.g., lymphomas), autoimmune rheumatic diseases (e.g., adult-onset Still's disease), granulomatous diseases, and miscellaneous causes.
Risk factors for FUO include advanced age, immunosuppression (due to HIV/AIDS, organ transplantation, or immunosuppressive therapy), recent travel to endemic areas, pre-existing comorbidities (such as diabetes and chronic kidney disease), and exposure to healthcare environments. Certain occupational exposures, close animal contact, and a history of intravenous drug use may increase the risk of specific etiologies within the FUO spectrum.
Patients with FUO typically present with persistent fever and constitutional symptoms such as malaise, weight loss, night sweats, and fatigue. The absence of focal symptoms or localizing signs complicates the diagnostic process. A thorough history including travel, occupational, and exposure history along with a meticulous physical examination, remains foundational. Evolution of new symptoms or signs during the course of the illness can provide crucial diagnostic clues.
The diagnostic evaluation of FUO is guided by a tiered approach. Initial investigations should include a comprehensive panel of laboratory tests (CBC, ESR/CRP, liver and renal function tests, blood cultures, urinalysis), basic chest radiography, and, when indicated, abdominal ultrasonography. Subsequent tailored testing is informed by clinical evolution and epidemiological context. Advanced imaging modalities such as CT, MRI, and PET/CT are increasingly utilized to detect occult infections, neoplasms, or inflammatory foci. Serological assays, autoantibody panels, and tissue biopsies (when indicated) are integral for specific diagnoses. The importance of avoiding empiric therapies prior to establishing a diagnosis is emphasized, as such interventions can obscure diagnostic findings.
Management of FUO is etiology-specific and should be deferred until a diagnosis is established, except in cases where the patient is critically ill or has rapidly deteriorating clinical status. Empiric therapy may be justified in select scenarios such as neutropenic fever, suspected temporal arteritis, or severe sepsis. Supportive care, including antipyretics for symptom control and management of underlying comorbidities, is essential. Multidisciplinary collaboration is often required, involving infectious disease specialists, rheumatologists, oncologists, and other experts as dictated by clinical findings.
Recent advances in molecular diagnostics, including next-generation sequencing and syndromic PCR panels, have enhanced the detection of rare pathogens and atypical presentations. The integration of PET/CT has significantly improved localization of occult inflammatory and neoplastic processes, particularly in cases where conventional imaging is non-revealing. Immunophenotyping and advanced serological assays are increasingly used to identify complex autoimmune and autoinflammatory syndromes. Ongoing research into host genetic susceptibility and immune profiling holds promise for the future personalization of FUO evaluation.
Contemporary guidelines advocate for a deliberate and stepwise diagnostic approach, emphasizing the importance of repeated clinical reassessment and avoidance of indiscriminate testing. The European Society of Clinical Microbiology and Infectious Diseases (ESCMID) and Infectious Diseases Society of America (IDSA) recommend a minimum set of core investigations, the judicious use of advanced imaging, and early multidisciplinary input. Empiric therapy should be restricted to selected high-risk scenarios, and unnecessary invasive procedures should be avoided unless strongly indicated by clinical or imaging findings.
The diagnostic evaluation of fever of unknown origin in adults necessitates a comprehensive, systematic, and evidence-based approach. Advances in diagnostic technologies and updated clinical guidelines have improved the identification of underlying etiologies, yet a significant proportion of cases remain unresolved. Clinicians must balance thorough investigation with judicious resource utilization, maintaining vigilance for emerging syndromes and rare disorders. Ongoing research and the integration of novel diagnostic modalities are expected to further refine the evaluation and management of FUO, ultimately improving patient outcomes and reducing the burden on healthcare systems.
