The assessment of placental function is pivotal in the management of high-risk pregnancies, where compromised placental activity can lead to serious fetal and maternal morbidity. This review synthesizes current clinical guidelines, recent advances, and evidence-based practices for placental evaluation in high-risk obstetric scenarios. Emphasis is placed on risk stratification, pathophysiological mechanisms, diagnostic modalities including Doppler ultrasonography, biochemical markers, and advanced imaging and their practical application in clinical decision-making. The article further discusses emergent research, guideline updates, and the integration of assessment strategies into routine prenatal care to optimize maternal-fetal outcomes.
High-risk pregnancies, characterized by a greater likelihood of adverse maternal or fetal outcomes, necessitate meticulous surveillance and functional assessment of the placenta. As the principal organ for fetal nourishment, gas exchange, and waste elimination, placental dysfunction is a common denominator in complications such as fetal growth restriction (FGR), preeclampsia, and stillbirth. Modern clinical practice increasingly relies on robust, evidence-driven protocols for placental evaluation to anticipate complications, facilitate timely intervention, and improve perinatal outcomes. This review critically appraises the contemporary approaches to placental functional assessment, highlighting their clinical relevance and integration into high-risk pregnancy management.
Placental insufficiency contributes significantly to the global burden of perinatal morbidity and mortality. FGR affects approximately 5–10% of pregnancies, with a substantial proportion attributable to deficient placental function. Preeclampsia complicates 2–8% of pregnancies worldwide, often coexisting with placental dysfunction. The resultant risks include preterm birth, neonatal intensive care admission, and long-term neurodevelopmental impairment. Early identification and targeted management of placental pathology are thus essential for reducing the burden and improving perinatal health indices.
Placental function is governed by complex mechanisms involving trophoblast invasion, spiral artery remodeling, and angiogenesis. Inadequate trophoblastic invasion or immune maladaptation can impair uteroplacental perfusion, leading to hypoxia and oxidative stress within the placental tissue. Such dysfunction manifests as impaired nutrient and oxygen transfer, altered hormone production, and endothelial dysfunction, forming the substrate for conditions like FGR and preeclampsia. Understanding these mechanisms underpins the rationale for targeted functional assessment and therapeutic intervention in high-risk pregnancies.
Several maternal, fetal, and placental risk factors predispose to placental insufficiency. Maternal factors include advanced maternal age, chronic hypertension, diabetes mellitus, autoimmune disorders, renal disease, and history of preeclampsia. Fetal factors such as chromosomal abnormalities and multiple gestations also elevate risk. Placental factors encompass abnormal implantation, previa, accreta spectrum, and prior placental abruption. Recognizing these risk factors is critical for timely assessment and surveillance planning.
Clinical manifestations of placental dysfunction are often subtle in early stages. Common indicators include fundal height lag, abnormal fetal growth trajectory on ultrasound, reduced fetal movements, and signs of maternal hypertension or proteinuria. In severe cases, features such as oligohydramnios, fetal distress, and abnormal fetal heart rate patterns may emerge. Early detection through vigilant clinical and imaging surveillance is essential for optimal maternal-fetal outcomes.
Placental functional assessment employs a multimodal approach. Doppler ultrasound remains the cornerstone, with uterine artery Doppler indices (e.g., pulsatility index, notching) predictive of preeclampsia and FGR. Umbilical artery Doppler evaluates fetoplacental resistance, while middle cerebral artery Doppler assists in identifying fetal adaptation to hypoxia. Biochemical markers such as placental growth factor (PlGF), soluble fms-like tyrosine kinase-1 (sFlt-1), and pregnancy-associated plasma protein A (PAPP-A) provide adjunctive information on placental health. Emerging modalities include three-dimensional power Doppler, MRI, and non-invasive placental perfusion imaging. The choice and frequency of assessments are tailored based on risk stratification and gestational age.
Management of placental dysfunction is multidisciplinary, encompassing maternal disease optimization, fetal surveillance, and judicious timing of delivery. Antihypertensive therapy, thromboprophylaxis, and corticosteroids for fetal lung maturity are employed as indicated. Serial Doppler and growth scans guide intervention thresholds. In cases of severe FGR or preeclampsia, early delivery may be warranted to mitigate maternal and fetal compromise. Neonatal care planning and multidisciplinary coordination are integral to care delivery in high-risk cases.
Recent years have seen the advent of advanced placental imaging techniques, including contrast-enhanced MRI and microvascular imaging, enabling earlier and more precise detection of functional impairment. Molecular biomarkers and proteomic profiling are under investigation for their potential to predict placental dysfunction before clinical manifestations. Artificial intelligence and machine learning algorithms are being developed to integrate multimodal data for risk prediction and personalized management. These advances hold promise for enhancing the sensitivity and specificity of placental assessment in high-risk pregnancies.
Current guidelines from organizations such as the American College of Obstetricians and Gynecologists (ACOG), International Society of Ultrasound in Obstetrics and Gynecology (ISUOG), and Royal College of Obstetricians and Gynaecologists (RCOG) advocate for individualized risk assessment and serial placental functional evaluation in women with risk factors. Routine uterine artery Doppler is recommended in the second trimester for women at increased risk, with escalation of surveillance in the presence of abnormal findings. Integration of biochemical markers is suggested for further stratification, while advanced imaging remains reserved for equivocal or severe cases. Multidisciplinary care pathways are emphasized for optimal outcomes.
Placental functional assessment is a cornerstone of high-risk pregnancy management, underpinning timely intervention and improved perinatal outcomes. Advances in imaging, biomarker discovery, and guideline-driven protocols have enhanced the precision and utility of placental evaluation. Continued research and integration of emerging technologies into clinical practice will further refine risk prediction and management strategies, ultimately reducing the burden of perinatal morbidity and mortality associated with placental dysfunction.
