Abstract

Precision physiology, particularly through the lens of individual oxygen-utilization profiles, has emerged as a transformative approach in optimizing clinical care and advancing personalized medicine. Understanding and harnessing the unique oxygen-consumption patterns of patients allows for more accurate diagnosis, risk stratification, and tailored therapeutic interventions across a spectrum of medical conditions. This review synthesizes recent literature regarding the epidemiology, pathophysiology, risk factors, clinical features, diagnosis, management, and emerging therapies centered on individual oxygen utilization, providing clinicians with actionable, evidence-based insights to enhance patient outcomes.

Introduction

The concept of precision physiology integrates personalized assessment of physiological parameters, of which oxygen-utilization profiling is a cornerstone. Oxygen is essential for cellular metabolism and systemic homeostasis; however, inter-individual variability in oxygen consumption and delivery influences disease presentation and therapeutic response. Leveraging individual oxygen-utilization profiles enables a nuanced understanding of metabolic health, exercise tolerance, and disease progression, thereby supporting a paradigm shift toward personalized medical interventions. This article reviews the scientific rationale, clinical relevance, and practical implications of adopting precision oxygen physiology in contemporary practice

Epidemiology / Disease Burden

Oxygen-utilization abnormalities underlie a broad array of acute and chronic conditions, including heart failure, chronic obstructive pulmonary disease (COPD), sepsis, and mitochondrial disorders. Epidemiological studies reveal that impaired oxygen extraction and utilization contribute to worse outcomes, increased hospitalization rates, and higher mortality across these populations. For example, in heart failure with reduced ejection fraction (HFrEF), reduced peak oxygen uptake (VO2 max) independently predicts mortality. Similarly, in critical care, failure to optimize tissue oxygen delivery and utilization increases the risk of multi-organ dysfunction. The burden of diseases affected by oxygen-utilization heterogeneity is significant, affecting millions globally and representing a substantial portion of healthcare resource utilization.

Pathophysiology

Oxygen delivery (DO2) to tissues is determined by cardiac output and arterial oxygen content, whereas oxygen utilization (VO2) depends on cellular extraction and mitochondrial function. Disruptions in any component ranging from impaired cardiac output, abnormal hemoglobin function, to mitochondrial dysfunction can result in inadequate oxygen utilization. Microvascular dysfunction, cytopathic hypoxia, and altered metabolic demand further modulate the oxygen-utilization profile. For example, in sepsis, mitochondrial impairment leads to a decoupling of oxygen delivery from utilization, resulting in cellular hypoxia despite adequate systemic oxygenation. Understanding these mechanisms is integral to identifying therapeutic targets and tailoring interventions.

Risk Factors

Several intrinsic and extrinsic factors influence individual oxygen-utilization profiles. Genetic predispositions affecting mitochondrial biogenesis, hemoglobinopathies, chronic inflammatory states, cardiorespiratory comorbidities, and aging are key intrinsic risk factors. Extrinsic contributors include sedentary lifestyle, malnutrition, environmental hypoxia, and medication-induced mitochondrial toxicity. Recognizing these risk factors is paramount for risk stratification and early intervention, particularly in populations at high risk for oxygen-utilization disorders, such as the elderly, patients with chronic cardiopulmonary disease, and critically ill individuals.

Clinical Features

Patients with impaired oxygen utilization often present with nonspecific symptoms such as exertional dyspnea, fatigue, exercise intolerance, and cognitive dysfunction. In advanced cases, features of end-organ hypoxia such as lactic acidosis, altered mental status, and organ dysfunction may predominate. Subtle impairments in oxygen utilization can precede overt clinical deterioration, underscoring the need for objective assessment tools. Variability in symptomatology is influenced by the underlying etiology, comorbid conditions, and compensatory physiological mechanisms.

Diagnosis

Diagnostic evaluation of individual oxygen-utilization profiles incorporates both non-invasive and invasive modalities. Cardiopulmonary exercise testing (CPET) remains the gold standard for quantifying VO2 max and characterizing exercise-limiting factors. Near-infrared spectroscopy (NIRS) enables real-time monitoring of tissue oxygenation at the bedside. Advanced imaging modalities, such as positron emission tomography (PET) and magnetic resonance spectroscopy (MRS), provide insights into regional oxygen consumption and mitochondrial function. Biomarkers, including lactate levels and mitochondrial DNA copy number, offer adjunctive diagnostic value. Integrating these assessments facilitates accurate phenotyping and informs individualized management strategies.

Treatment & Management

Management strategies are tailored to address the specific pathophysiological derangements underlying impaired oxygen utilization. Interventions may include optimization of cardiac output via pharmacotherapy or device therapy, correction of anemia, targeted rehabilitation programs to enhance aerobic capacity, and metabolic modulation through mitochondrial-targeted agents. In critical care settings, individualized oxygen targets and titration of supplemental oxygen are increasingly adopted to avoid both hypoxemia and hyperoxia. Nutritional optimization and physical activity interventions play pivotal roles in improving tissue oxygen utilization, particularly in chronic disease populations.

Recent Advances / Emerging Therapies

Recent advances have focused on leveraging big data analytics, machine learning, and wearable biosensors to generate real-time, individualized oxygen-utilization profiles. Mitochondrial-targeted therapeutics, such as coenzyme Q10 analogs and PGC1-alpha modulators, are being investigated in clinical trials for their potential to enhance cellular oxygen utilization. Precision exercise prescriptions and remote monitoring platforms now enable dynamic, patient-specific optimization of functional capacity. The integration of genomics and metabolomics is poised to further refine risk stratification and therapeutic targeting.

Guideline Recommendations

International guidelines increasingly endorse the incorporation of individualized physiological assessment in the management of cardiorespiratory and metabolic disorders. The European Society of Cardiology and American Heart Association recommend routine assessment of VO2 max in selected populations and highlight the importance of tailoring oxygen therapy to individual needs. Guidelines advocate for a multimodal approach, integrating clinical, functional, and molecular data to guide personalized interventions. Ongoing updates reflect the evolving evidence base supporting precision physiology.

Conclusion

Precision physiology through individual oxygen-utilization profiling represents a paradigm shift in patient care, offering opportunities to improve diagnostic accuracy, optimize therapy, and enhance outcomes across diverse clinical settings. As technology and scientific understanding advance, the integration of individualized oxygen-consumption assessment into routine practice is likely to become standard of care, fostering a more nuanced and effective approach to disease management. Continued research and guideline refinement will be essential to fully realize the promise of precision oxygen physiology in modern medicine.
