The management of patients undergoing assisted reproductive technologies (ART) requires a comprehensive, multidisciplinary approach that spans preconception assessment, diagnostic work-up, therapeutic interventions, and ongoing support through pregnancy and postpartum. This review synthesizes up-to-date evidence and guideline recommendations, emphasizing mechanisms underlying infertility, epidemiological trends, risk stratification, diagnostic modalities, treatment algorithms, and recent innovations. We provide a framework for optimizing patient outcomes across the ART continuum, discuss individualized care based on risk profiles, and highlight clinical and practical implications for reproductive endocrinologists and fertility specialists.
Infertility is a pervasive clinical challenge, affecting approximately 10-15% of couples worldwide. Advances in assisted reproductive technologies (ART), such as in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI), have transformed management strategies and improved the prognosis for many patients. However, the complexity of infertility necessitates a holistic approach, incorporating precise etiological diagnosis, evidence-based interventions, and ongoing psychosocial support. The continuum of assisted reproductive care extends from initial evaluation to long-term follow-up, making continuity and coordination of care critical for optimizing success rates and minimizing risks.
The global burden of infertility has risen, with recent estimates indicating over 48 million couples worldwide affected. Both male and female factors contribute almost equally, though geographic, socioeconomic, and cultural variances exist. Age-related decline in female fertility, increasing prevalence of lifestyle-related disorders such as obesity and metabolic syndrome, and environmental exposures have all contributed to the rising demand for ART. Societal trends toward delayed childbearing further amplify this burden, leading to increased utilization of fertility services and ART cycles globally, particularly in high-income countries.
Infertility arises from diverse etiologies including ovulatory dysfunction (e.g., polycystic ovary syndrome, hypothalamic amenorrhea), tubal disease (e.g., pelvic inflammatory disease, endometriosis), uterine factors (e.g., fibroids, congenital anomalies), and male factors (e.g., oligozoospermia, azoospermia, sperm motility defects). Molecular and cellular mechanisms implicated in ART outcomes include oocyte and sperm quality, endometrial receptivity, embryo competence, and immunological factors. Understanding the interplay between these biological determinants and patient-specific characteristics informs tailored management strategies.
Common risk factors for infertility include advanced maternal age, obesity, smoking, excessive alcohol consumption, sexually transmitted infections, environmental toxins, and genetic predispositions. Additionally, iatrogenic risks, such as those associated with cancer therapy or pelvic surgery, may impact reproductive potential. ART-specific risks, including ovarian hyperstimulation syndrome (OHSS), multiple pregnancy, and perinatal complications, further underscore the need for individualized risk assessment throughout the care continuum.
The clinical presentation of infertility is often insidious and may be recognized only after a period of unsuccessful conception. Women may present with menstrual irregularities, signs of androgen excess, pelvic pain, or a history of pelvic infection or endometriosis. Men may report erectile or ejaculatory dysfunction, prior testicular trauma, or a family history of infertility. Comprehensive evaluation necessitates a detailed reproductive, medical, and surgical history, supplemented by focused physical examination and psychosocial assessment.
Diagnostic work-up is multifaceted and should be initiated after 12 months of unprotected intercourse (or earlier in women over 35 years or with known risk factors). Female assessment includes hormone profiling (FSH, LH, estradiol, AMH), ovulation monitoring, transvaginal ultrasonography, and uterine/tubal patency evaluation via hysterosalpingography or sonohysterography. Male evaluation encompasses semen analysis (concentration, motility, morphology), hormonal assays, and, when indicated, genetic testing or testicular biopsy. Advanced diagnostics, such as preimplantation genetic testing (PGT), endometrial receptivity assays, and sperm DNA fragmentation testing, may be warranted in select cases.
Management strategies are guided by the underlying etiology, patient age, duration of infertility, and prior treatment responses. First-line interventions may include lifestyle modification, ovulation induction (e.g., clomiphene citrate, letrozole), and intrauterine insemination (IUI). ART modalities such as IVF/ICSI are considered for tubal disease, severe male factor infertility, or failed first-line therapy. Treatment protocols must be tailored to minimize OHSS risk, optimize embryo selection, and address comorbidities. Comprehensive counseling regarding success rates, risks, and alternative options (e.g., donor gametes, surrogacy, adoption) is essential. Timely referral to specialized centers and ongoing psychosocial support enhance adherence and patient satisfaction.
Emerging technologies in ART include time-lapse embryo imaging, noninvasive embryo assessment, artificial intelligence-driven embryo selection, and vitrification-based cryopreservation of gametes and embryos. Expanded genetic screening, such as PGT-A for aneuploidy and PGT-M for monogenic diseases, improves implantation rates and reduces miscarriage risk. Ovarian tissue cryopreservation offers fertility preservation for cancer survivors. Investigational therapies, such as stem cell-derived gametes and mitochondrial replacement, hold promise but require further validation. Advances in luteal phase support, adjuvant therapies (e.g., growth hormone, immunomodulators), and individualized ovarian stimulation protocols are refining ART outcomes.
Professional societies such as ESHRE, ASRM, and NICE provide evidence-based guidelines for ART practice. Key recommendations include early referral for women over 35 or with risk factors, individualized ovarian stimulation protocols, single embryo transfer to minimize multiple gestations, comprehensive counseling on risks and alternatives, and the use of adjunctive technologies based on clinical indication. Shared decision-making and multidisciplinary care are emphasized, with a focus on safety, efficacy, and patient-centered outcomes.
The landscape of assisted reproductive care is rapidly evolving, driven by advances in diagnostic technologies, therapeutic options, and personalized medicine. Effective patient management across the ART continuum necessitates a nuanced understanding of underlying pathophysiology, risk stratification, and evidence-based interventions. Clinicians must remain vigilant to the unique needs and preferences of each patient, ensuring holistic, guideline-concordant, and ethically sound care. Ongoing research and innovation will continue to enhance outcomes and expand possibilities for individuals and couples seeking to build families through ART.
