A thorough patient history and physical examination, supplemented with appropriate laboratory and imaging studies, can help pinpoint the etiology.
A systematic diagnostic approach is essential. Initial laboratory workup should include a complete blood count, metabolic panel, stool studies, and tests for malabsorption. Advanced imaging or endoscopic evaluations may be necessary in certain cases. The aim should be to identify treatable causes and to avoid unnecessary invasive procedures.
Management should be tailored according to the underlying cause. For instance, inflammatory bowel diseases may require immunomodulatory therapy, while infectious causes necessitate antimicrobial treatment. Dietary modifications and symptomatic relief using antidiarrheal agents form the cornerstone of therapy in most cases.
Emerging evidence supports the role of probiotics in managing chronic diarrhea, particularly in cases of antibiotic-associated and infectious diarrhea. However, more robust clinical trials are needed to establish their definitive role.
Chronic diarrhea poses a significant burden on healthcare due to its prevalence and potential complications. A comprehensive and systematic approach to its diagnosis and management is crucial in clinical practice. Further research is warranted to explore novel therapeutic strategies, such as the role of the gut microbiota and probiotics, in managing this complex condition.
