Enteric fever, commonly known as typhoid fever, is a life-threatening illness caused by Salmonella Typhi. Despite significant strides in global healthcare, it continues to pose a significant burden, particularly in developing countries. This article aims to shed light on the recent advancements in the diagnosis and treatment of enteric fever.
Early and accurate diagnosis of enteric fever is crucial to prevent complications and reduce mortality. Traditional culture methods, despite being the gold standard, are time-consuming and have low sensitivity. Recent advancements have led to the development of rapid diagnostic tests (RDTs) such as Tubex TF and Typhidot. These RDTs, based on detecting IgM and IgG antibodies, provide results within hours and have shown promising sensitivity and specificity.
The treatment of enteric fever has been complicated by the increasing resistance of Salmonella Typhi to conventional antibiotics. The recent introduction of newer generation antibiotics, such as azithromycin and ceftriaxone, has shown promising results. Additionally, the use of fluoroquinolones, despite resistance issues, remains a viable option in many regions. The choice of antibiotic should be guided by local resistance patterns.
Vaccination remains a key strategy in the management of enteric fever. While the currently available vaccines (Ty21a and Vi polysaccharide) provide moderate protection, the newer conjugate vaccine (Typbar-TCV) has demonstrated higher efficacy and longer duration of protection. Furthermore, improvements in sanitation and provision of safe drinking water are crucial to prevent the spread of the disease.
While enteric fever continues to be a significant health challenge, advancements in diagnosis and treatment are paving the way for better management. Rapid diagnostic tests, newer antibiotics, and more effective vaccines are transforming the landscape of enteric fever management. However, it is crucial to continue research and development efforts to further improve these strategies and ultimately eradicate the disease.
