Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality among women worldwide, and its impact is particularly significant during the reproductive years. Preconception cardiovascular risk assessment and prevention strategies are critical to optimize both maternal and fetal outcomes. This review synthesizes current evidence, elucidates mechanisms linking pre-pregnancy cardiovascular health with pregnancy outcomes, and provides clinically actionable recommendations for early identification and management of modifiable risk factors in women considering pregnancy. Recent guideline updates and emerging therapies are discussed to enhance the implementation of preventive cardiology in preconception care.
The intersection of cardiovascular health and reproductive medicine is increasingly recognized as a crucial area for improving long-term outcomes in women. Cardiovascular complications are a significant contributor to maternal morbidity and adverse perinatal outcomes. Preconception represents a unique window for risk stratification and primary prevention, as many cardiovascular risk factors are modifiable and amenable to intervention prior to pregnancy. This review aims to provide a comprehensive, evidence-based overview of cardiovascular prevention strategies for women before pregnancy, drawing upon recent research, clinical guidelines, and expert consensus.
Globally, CVD accounts for over one-third of deaths in women, with an increasing burden observed in younger age groups due to rising prevalence of obesity, diabetes, and metabolic syndrome. Pregnancy itself is a cardiovascular stress test, unmasking or exacerbating underlying risk factors. Epidemiological studies have demonstrated that up to 10% of pregnancies are complicated by hypertensive disorders, and women with pre-existing cardiovascular risk factors have a higher incidence of adverse outcomes such as preeclampsia, gestational diabetes, and preterm birth. The repercussions extend beyond pregnancy, with women experiencing pregnancy complications facing elevated long-term cardiovascular risk.
The pathophysiological mechanisms underpinning CVD in the preconception and peripartum periods are multifactorial. Insulin resistance, chronic inflammation, endothelial dysfunction, and dyslipidemia may precede pregnancy and are exacerbated by the hemodynamic and metabolic demands of gestation. Genetic predisposition, autoimmune processes, and hormonal changes further modulate cardiovascular risk. The interplay between these factors can result in vascular maladaptation, impaired placentation, and increased susceptibility to acute cardiac events during pregnancy.
Traditional cardiovascular risk factors such as hypertension, dyslipidemia, diabetes mellitus, obesity, and smoking are highly prevalent among women of reproductive age. Additional risk factors include polycystic ovary syndrome, autoimmune diseases (e.g., systemic lupus erythematosus, antiphospholipid syndrome), family history of premature CVD, and adverse pregnancy outcomes in previous gestations. Socioeconomic status, ethnicity, and psychosocial stressors also contribute to risk stratification. Comprehensive preconception assessment should incorporate both traditional and sex-specific risk factors.
Women with cardiovascular risk factors may be asymptomatic prior to pregnancy. However, subtle clinical features such as elevated blood pressure, abnormal lipid profiles, impaired glucose tolerance, and evidence of subclinical atherosclerosis (e.g., carotid intima-media thickness, coronary artery calcium scoring) can be detected through targeted screening. In high-risk populations, early markers of endothelial dysfunction or microalbuminuria may also be present. Recognizing these features before conception allows for timely intervention and tailored management.
Diagnosis of cardiovascular risk in the preconception setting involves a combination of clinical assessment, laboratory investigations, and, in selected cases, advanced imaging. Blood pressure measurement, fasting lipid panel, hemoglobin A1c, and screening for renal function are standard components of preconception evaluation. For women with known risk factors or prior adverse pregnancy outcomes, echocardiography, ambulatory blood pressure monitoring, and assessment of markers of inflammation or thrombophilia may be warranted. Risk calculators, such as the ASCVD Risk Estimator, can aid in quantifying short- and long-term risk, though their application in younger women requires clinical judgment.
Management of cardiovascular risk factors prior to pregnancy focuses on lifestyle modification and, when indicated, pharmacologic therapy. Weight optimization through diet and exercise, smoking cessation, and management of comorbidities are foundational interventions. Pharmacologic treatment of hypertension, dyslipidemia, and diabetes should be tailored to agents with established safety profiles in women planning pregnancy. Preconception counseling should address medication teratogenicity and the importance of strict glycemic and blood pressure control. Multidisciplinary collaboration involving cardiology, obstetrics, and primary care enhances individualized care and optimizes outcomes.
Recent advances in cardiovascular prevention include the adoption of novel lipid-lowering agents, such as PCSK9 inhibitors, and the potential use of SGLT2 inhibitors and GLP-1 receptor agonists for cardiometabolic risk reduction in women of childbearing age. Advances in risk stratification, including the use of biomarkers and genetic testing, are under investigation. Digital health interventions, such as mobile applications and remote monitoring, offer promising tools for lifestyle modification and adherence to preventive strategies. Ongoing research is evaluating the role of preconception interventions in improving both short- and long-term cardiovascular outcomes.
Current guidelines from the American Heart Association, American College of Obstetricians and Gynecologists, and European Society of Cardiology emphasize the importance of preconception cardiovascular risk assessment and optimization. Recommendations include routine screening for hypertension, dyslipidemia, diabetes, and lifestyle factors in all women of reproductive age. Women with pre-existing CVD or high-risk conditions should receive specialized counseling and multidisciplinary management. Individualized risk assessment and patient-centered decision-making are paramount, with a focus on optimizing modifiable risk factors before pregnancy.
Cardiovascular prevention in women before pregnancy represents a pivotal opportunity to improve maternal and fetal health. Early identification and management of risk factors, guided by current evidence and multidisciplinary collaboration, can mitigate adverse pregnancy outcomes and reduce long-term cardiovascular morbidity. Continued research, education, and guideline implementation are essential to integrate preventive cardiology into routine preconception care and achieve lasting benefits for women\'s cardiovascular health.
