Recent guidelines and advances are discussed to inform best practices for multidisciplinary teams in optimizing patient recovery trajectories.
Effective monitoring and timely intervention are fundamental to achieving early clinical recovery, particularly in complex or chronic conditions. Traditional physician-centered models are increasingly complemented by nurse-led initiatives, leveraging the unique skill set of nursing professionals for continuous patient engagement. Nurse-led longitudinal monitoring, underpinned by case-based learning, offers a dynamic and responsive framework that can facilitate early detection of clinical deterioration, promote adherence, and reduce hospital readmissions. This article examines the rationale, mechanisms, and clinical application of such models, with an emphasis on recent evidence and guideline-based recommendations.
Chronic diseases such as heart failure, diabetes, COPD, and oncological conditions collectively account for the majority of global morbidity, mortality, and healthcare utilization. Early recovery and prevention of complications are crucial to reducing disease burden. Nurse-led longitudinal monitoring has demonstrated significant potential in mitigating these burdens. Contemporary clinical guidelines increasingly endorse nurse-led monitoring as a cornerstone of post-discharge and chronic disease management.
