Safe and accessible surgical care is a cornerstone of comprehensive health systems, yet global disparities persist, particularly in low- and middle-income countries (LMICs). This review synthesizes current public health strategies aimed at expanding access to safe surgical care, emphasizing epidemiological trends, underlying mechanisms, risk factors, clinical challenges, diagnostic barriers, management approaches, emerging innovations, and guideline-based recommendations. The article integrates recent evidence and expert insights, focusing on the practical implications for health professionals involved in surgical systems strengthening and global health policy.
Surgical conditions account for a significant proportion of global morbidity and mortality, with an estimated 5 billion people lacking access to safe, affordable surgical and anesthesia care when needed. The Lancet Commission on Global Surgery highlighted the urgent need to prioritize surgical care as a public health intervention. Inadequate surgical infrastructure, workforce shortages, and inequitable distribution of resources underscore the necessity for comprehensive public health strategies. This review aims to inform clinicians, health policymakers, and global health stakeholders by presenting a scientific overview of the key elements and recent advances in expanding access to safe surgical care systems.
The global burden of surgical disease encompasses trauma, malignancy, congenital anomalies, obstetric emergencies, and infectious conditions requiring operative intervention. Recent estimates attribute approximately 30% of the global burden of disease to conditions treatable by surgery. LMICs bear a disproportionate share of unmet surgical needs, with perioperative mortality rates up to seven times higher than in high-income countries. Delayed or absent surgical care leads to preventable death and disability, significantly impacting economic productivity and quality of life.
The pathophysiological basis of surgical disease spans acute and chronic processes, ranging from traumatic hemorrhage and sepsis to neoplastic progression and organ dysfunction. Delays in surgical intervention exacerbate physiological derangements, increase complication rates, and diminish the likelihood of favorable outcomes. Inadequate infection control and perioperative monitoring contribute to adverse events and surgical site infections, underscoring the need for system-level reforms in the delivery of safe surgical care.
Barriers to accessing safe surgical care are multifactorial. Key risk factors include geographic isolation, poverty, gender inequity, limited health literacy, and systemic underfunding of surgical services. Health system constraints such as workforce shortages, poorly equipped facilities, unreliable supply chains, and lack of essential medicines further limit access. Vulnerable populations—children, women, the elderly, and those with disabilities—face heightened risks of adverse outcomes due to delayed or suboptimal care.
Patients presenting with surgical disease often exhibit late-stage or complicated manifestations secondary to delayed access. Typical clinical features include advanced infection, sepsis, hemorrhagic shock, or metastatic disease. In obstetric care, untreated obstructed labor can lead to maternal and neonatal mortality. The clinical spectrum is broad, requiring providers to maintain a high index of suspicion and prompt recognition, particularly in resource-limited settings where diagnostic adjuncts may be scarce.
Timely and accurate diagnosis of surgical conditions remains a critical challenge, especially in remote or under-resourced environments. Key diagnostic limitations include lack of imaging modalities, laboratory support, and specialist expertise. Point-of-care diagnostics, telemedicine, and the implementation of standardized triage protocols have shown promise in mitigating diagnostic delays. Training non-physician clinicians in basic surgical assessment can further enhance early identification and referral pathways.
Effective management of surgical conditions hinges on an integrated approach encompassing preoperative assessment, safe anesthesia, operative technique, and postoperative care. WHO\'s Safe Surgery Saves Lives checklist and protocols for infection prevention are widely endorsed to reduce perioperative complications. Task-shifting approaches—training non-specialist providers to perform basic surgical procedures—have demonstrated safety and efficacy in many LMICs. Strengthening supply chains, ensuring availability of essential medicines, and establishing referral networks are vital for comprehensive surgical care delivery.
Innovative models such as mobile surgical units, tele-surgery, and eHealth platforms are bridging gaps in surgical access. Global surgery initiatives, including National Surgical, Obstetric, and Anesthesia Plans (NSOAPs), support strategic system strengthening. Advances in low-cost technologies, such as portable ultrasound and laparoscopic kits, are expanding the reach of minimally invasive procedures. Capacity-building partnerships and global health fellowships are fostering workforce development and knowledge exchange, improving local expertise and sustainability.
International guidelines emphasize the integration of surgical care into Universal Health Coverage (UHC) frameworks. The WHO and World Bank recommend that essential surgical services be available at the district hospital level and accessible within two hours’ travel for all populations. The Safe Surgery Saves Lives campaign outlines critical safety standards. National and international bodies endorse continuous workforce training, data-driven quality improvement, and investment in surgical infrastructure as core strategies. Advocacy for equitable surgical care is increasingly recognized as a human rights imperative.
Expanding access to safe surgical care requires a coordinated, systems-based approach addressing epidemiological, infrastructural, and socio-economic determinants. Evidence-based public health strategies—ranging from workforce training and guideline implementation to the adoption of innovative technologies—are essential to narrowing the global surgical gap. Ongoing research, collaborative partnerships, and policy engagement will be crucial in achieving equitable surgical care for all, ultimately reducing preventable morbidity and mortality worldwide.
