Preserving functional independence in older adults undergoing surgery has become a clinical priority, given the growing population of geriatric patients and their unique vulnerability to perioperative complications. This article reviews the rationale, evidence base, and practical approaches to implementing functional-preservation strategies in age-adapted surgery. It explores the burden of surgical disease in the elderly, underlying pathophysiological changes, risk factors for poor outcomes, clinical features that influence perioperative risk, diagnostic considerations, and comprehensive management strategies. Recent advances, guideline-based recommendations, and future directions for optimizing care and improving postoperative trajectories in geriatric surgical patients are critically discussed.
The demographic transition toward an aging society has dramatically increased the number of older individuals requiring surgical interventions. Traditional surgical paradigms often fail to account for the physiological heterogeneity and functional reserve of geriatric patients, leading to suboptimal outcomes. Modern geriatric medicine emphasizes the importance of functional preservation, prioritizing the maintenance of independence and quality of life over mere survival. Age-adapted surgical strategies integrate geriatric assessment, multidisciplinary care, and tailored perioperative protocols to reduce complications, facilitate recovery, and prevent functional decline. This review synthesizes current evidence and expert consensus on implementing functional-preservation strategies in geriatric surgery, highlighting the clinical imperatives and research advances shaping this field.
Globally, the proportion of individuals aged 65 and older is rising, with projections estimating over 1.5 billion elderly people by 2050. Surgical admissions in this age group have increased correspondingly, accounting for up to 40% of inpatient procedures in high-income countries. Geriatric patients present a heavier disease burden, characterized by multimorbidity, frailty, and polypharmacy. Postoperative complications, prolonged hospital stays, rehospitalization, and functional decline are significantly more prevalent in this cohort. Notably, up to one-third of older adults experience new or worsened disability following surgery, underscoring the urgent need for preventive and rehabilitative strategies.
Aging is associated with diverse physiological alterations that impact surgical risk and recovery. Reduced cardiac and pulmonary reserve, diminished renal and hepatic function, altered immune response, and impaired musculoskeletal integrity collectively lower the threshold for perioperative complications. Neurocognitive vulnerability, including the risk for postoperative delirium and cognitive dysfunction, further complicates care. These pathophysiological changes are compounded by the prevalence of sarcopenia, frailty, and impaired nutritional status, which adversely affect wound healing, stress tolerance, and overall resilience. Understanding these mechanisms is essential for designing interventions that preserve functional capacity.
Key risk factors for poor surgical outcomes in geriatric patients include advanced age, frailty, pre-existing cognitive impairment, functional dependence, malnutrition, polypharmacy, and the presence of multiple comorbidities such as cardiovascular disease, diabetes, and chronic kidney disease. Operative factors such as emergency procedures, high surgical complexity, and prolonged anesthesia further increase risk. Preoperative identification and stratification of these risk factors using validated tools (e.g., the Clinical Frailty Scale, Charlson Comorbidity Index, and surgical risk calculators) enable targeted interventions and informed shared decision-making.
Clinical assessment of the geriatric surgical patient extends beyond standard medical evaluation. Comprehensive geriatric assessment (CGA) encompasses functional status (activities of daily living, instrumental activities), cognitive function, mood, nutritional status, mobility, social support, and comorbidities. Attention to atypical presentations, such as delirium, falls, and incontinence, is critical. The interplay between baseline function and surgical stress determines the risk of functional decline. Early detection of vulnerabilities permits proactive management and tailored perioperative planning.
Diagnostic evaluation in older adults must account for age-related physiological changes that may mask or alter disease presentation. Laboratory and imaging findings should be interpreted in the context of baseline function and comorbidities. Frailty assessment, cognitive screening (e.g., Mini-Cog, Montreal Cognitive Assessment), nutritional evaluation (e.g., Mini Nutritional Assessment), and physical performance measures (e.g., gait speed, grip strength) are integral to preoperative workup. Multidimensional risk stratification informs prognosis and guides the intensity of perioperative support.
Management strategies in geriatric surgery should be individualized, with an emphasis on preserving autonomy and minimizing harm. Prehabilitation targeted interventions to enhance physical, nutritional, and psychological resilience prior to surgery has demonstrated efficacy in improving postoperative outcomes. Multimodal analgesia, delirium prevention protocols, and early mobilization are cornerstones of perioperative care. Multidisciplinary involvement, including geriatricians, anesthesiologists, surgeons, physiotherapists, and nutritionists, optimizes management. Postoperative rehabilitation and transitional care planning are essential to maintain functional gains and prevent readmission or institutionalization.
Recent research has focused on the development and validation of predictive models for geriatric surgical risk, integration of frailty metrics into perioperative pathways, and the use of minimally invasive surgical techniques to reduce physiological stress. Enhanced Recovery After Surgery (ERAS) protocols have been adapted to geriatric populations, emphasizing early mobilization, nutritional optimization, and reduction of opioid use. Digital health tools such as wearable activity monitors and telemedicine for postoperative follow-up are emerging as valuable adjuncts for functional monitoring and patient engagement. Ongoing clinical trials are evaluating novel pharmacological and non-pharmacological interventions to mitigate delirium, sarcopenia, and cognitive decline.
International guidelines underscore the importance of CGA, functional assessment, and shared decision-making in older surgical patients. The American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) Geriatric Surgery Guidelines and the European Society of Anaesthesiology and Intensive Care (ESAIC) recommend routine frailty screening, delirium prevention, and multidisciplinary perioperative care. Evidence-based protocols for pain management, early mobilization, and nutritional support are emphasized. Guidelines advocate for patient-centered goals of care discussions, particularly in high-risk or elective procedures, to align interventions with individual preferences and values.
Functional-preservation strategies represent a paradigm shift in geriatric surgery, moving beyond mortality reduction to prioritize independence and quality of life. Age-adapted surgical care, informed by comprehensive assessment, evidence-based protocols, and multidisciplinary collaboration, mitigates perioperative risks and promotes recovery in older adults. As the population ages, continued research, innovation, and guideline development are needed to refine approaches and ensure optimal outcomes for geriatric surgical patients.
