Chronic cough can be attributed to a variety of conditions such as gastroesophageal reflux disease (GERD), asthma, upper airway cough syndrome (UACS), and non-asthmatic eosinophilic bronchitis (NAEB). It is crucial to conduct a detailed clinical evaluation, including a thorough history and physical examination, to identify potential triggers and underlying conditions. Diagnostic tests such as spirometry, chest radiographs, and bronchoscopy can be used judiciously based on clinical suspicion.
Treatment of chronic cough should be tailored to the identified cause. For GERD-related cough, lifestyle modifications and proton pump inhibitors are first-line treatments. Inhaled corticosteroids are the treatment of choice for cough-variant asthma and NAEB, while UACS can be managed with first-generation antihistamines and nasal steroids. Empiric treatment trials can be considered when the etiology is unclear, but should be guided by the most likely diagnosis based on clinical features.
Despite a comprehensive approach, chronic cough remains a therapeutic challenge due to the lack of specific diagnostic markers and the potential for multiple concurrent etiologies. Future research should focus on identifying novel biomarkers and developing targeted therapies. It is also important to improve our understanding of the pathophysiology of chronic cough, which may lead to more effective treatment strategies.
In conclusion, chronic cough requires a meticulous, patient-centered approach for effective diagnosis and management. While several challenges persist, ongoing research and innovation hold promise for improving patient outcomes. Through a better understanding of the etiology and pathophysiology of chronic cough, clinicians can enhance their diagnostic acumen and therapeutic efficacy.
