Immunotherapy, a revolutionary approach to cancer treatment, is transforming the oncological landscape. As physicians, understanding the intricacies of these therapies is imperative to ensure optimal patient care.
Immunotherapy harnesses the body's immune system to fight cancer. It does this in two ways: by stimulating the immune system to work harder or by providing it with artificial immune system proteins. The main types include checkpoint inhibitors, CAR T-cell therapy, and cancer vaccines.
These drugs, such as pembrolizumab and nivolumab, block immune checkpoints, proteins that prevent immune cells from attacking non-harmful cells in the body. By doing so, they boost the immune response against cancer cells.
Chimeric Antigen Receptor (CAR) T-cell therapy involves modifying a patient's T cells to express a specific receptor that can bind to a certain protein on cancer cells, enhancing the immune system's ability to target and destroy these cells.
Cancer vaccines, like the Sipuleucel-T for prostate cancer, work by stimulating the immune system to attack specific cancer cells. These vaccines can be either preventive, like the HPV vaccine, or therapeutic, like the Bacillus Calmette-Guérin (BCG) vaccine for bladder cancer.
Immunotherapy can cause side effects, often resembling flu-like symptoms. However, severe immune-mediated adverse events can occur. Physicians must be adept at recognizing these and managing them effectively, often requiring multidisciplinary collaboration.
Immunotherapy represents a significant shift in cancer treatment, offering new hope to patients. As physicians, it is crucial to stay updated on these therapies, their mechanisms of action, and their management to provide the best possible care to our patients.
