Abstract

The intersection of surgical innovation and addiction recovery has garnered increasing interest, particularly with the advent of functional recovery–centered surgical approaches. This review synthesizes recent evidence and clinical guidelines regarding the role of surgery in augmenting addiction recovery, with an emphasis on neurobiological mechanisms, risk stratification, and outcomes. It evaluates the epidemiological landscape, pathophysiology, clinical features, diagnostic criteria, and management strategies for substance use disorders within surgical contexts, highlighting the potential and limitations of emerging therapies. Multidisciplinary collaboration, patient selection, and functional outcome assessment are discussed to inform best practices for integrating surgical interventions in addiction medicine.

Introduction

Addiction remains a pervasive and complex health challenge, with major implications for morbidity, mortality, and healthcare systems globally. Traditionally managed through pharmacological and behavioral interventions, the role of surgery—particularly functional recovery–centered surgical approaches—is evolving. These approaches aim not only to disrupt pathological neurocircuitry but also to optimize post-surgical functional outcomes relevant to addiction remission. Epidemiology / Disease Burden

Recurrent relapses and chronicity characterize SUDs, leading to profound personal, social, and economic impact. Current clinical guidelines emphasize a multidisciplinary approach to addiction recovery, recommending surgical intervention only within specialized centers and in the context of rigorous ethical oversight. Patient selection should be stringent, prioritizing those with severe, treatment-resistant addiction who have exhausted conventional therapies. Comprehensive preoperative evaluation, informed consent, and long-term follow-up are mandated. Guideline committees advocate for continued research and registry participation to inform evidence-based practice and optimize patient safety.

Functional recovery–centered surgical approaches represent a promising frontier in the management of refractory substance use disorders. By targeting the neurobiological substrates of addiction, these interventions offer hope for individuals who have failed standard treatments. Nevertheless, their application requires meticulous patient selection, robust clinical governance, and integration with holistic rehabilitation paradigms. Ongoing research is essential to elucidate long-term outcomes, refine patient selection, and establish consensus guidelines. With careful implementation, surgical innovation may significantly enhance the trajectory of addiction recovery and reduce the global burden of substance use disorders.
