Common causes of unexplained weight loss include malignancy, gastrointestinal diseases, endocrine disorders, and psychiatric conditions. Malignancies, particularly of the lung, pancreas, and gastrointestinal tract, are the most common serious causes. Malabsorption syndromes, peptic ulcer disease, and inflammatory bowel disease are among the gastrointestinal causes. Endocrine disorders, such as hyperthyroidism and diabetes mellitus, can also lead to weight loss. Depression and other psychiatric disorders are often overlooked causes.
After the initial evaluation, laboratory tests and imaging studies should be guided by the clinical suspicion. Basic tests include complete blood count, metabolic panel, thyroid function tests, and urinalysis. Depending on the patient's history and physical examination, further investigations may include endoscopy, computed tomography, or psychiatric evaluation.
Treatment should be directed at the underlying cause. In many cases, this may involve a multidisciplinary approach involving oncologists, gastroenterologists, endocrinologists, and psychiatrists. Regular follow-up is essential to monitor the patient's response to treatment and to detect any new symptoms or signs.
Unexplained weight loss is a challenging clinical problem that requires a thorough and systematic approach. By understanding the common causes and adopting a comprehensive diagnostic strategy, physicians can effectively manage these patients and improve their outcomes.
