Gastrointestinal bleeding (GIB) poses a diagnostic challenge due to its diverse etiology, variable presentation, and potential for significant morbidity and mortality. A systematic approach to diagnosis and management is critical for optimal patient outcomes.
Assessment of GIB begins with a comprehensive history and physical examination. Key considerations include the rate and volume of bleeding, presence of comorbid conditions, use of anticoagulants, and previous history of GIB. The physical examination should focus on hemodynamic stability and signs of chronic liver disease, which may suggest variceal bleeding.
The choice of diagnostic modality depends on the suspected source of bleeding. Upper endoscopy is the first-line investigation for upper GIB, with a sensitivity and specificity of over 90%. For suspected lower GIB, colonoscopy is preferred. However, in cases of ongoing, brisk bleeding, angiography may be more appropriate. Capsule endoscopy and enteroscopy can be useful for obscure GIB after negative upper and lower endoscopies.
Radiologic investigations play a crucial role in the diagnostic algorithm of GIB. Computed tomography angiography (CTA) can identify active bleeding and localize its source, especially in lower GIB. Nuclear scintigraphy may be considered for slow bleeding rates when other modalities are non-diagnostic.
Management strategies for GIB are guided by the underlying etiology, severity of bleeding, and patient's overall clinical status. Resuscitation, including volume replacement, is paramount. Pharmacologic agents to reduce acid secretion are commonly used. Endoscopic therapy remains the mainstay of treatment for most causes of GIB. Surgical or radiological intervention may be required in refractory cases.
Deciphering the diagnostic approach to GIB requires a methodical evaluation and tailored use of available diagnostic modalities. Understanding the strengths and limitations of each modality, along with an appreciation of patient-specific factors, is key to effective management and improved patient outcomes.
